Objective:To explore the effectiveness of early enteral nutrition therapy in ICU patients with respiratory failure.Methods:A total of 76 ICU patients with respiratory failure,admitted from May 2024 to May 2025,were included in the study.They were divided into an observation group and a control group using a random number table method,and relevant treatment indicators were compared.Results:The total effective rate in the observation group was higher than that in the control group(p<0.05).After treatment,the observation group showed superior lung function,organ function,health status,nutritional status,body mass index,and blood gas analysis indicators compared to the control group(p<0.05).Conclusion:Early enteral nutrition therapy is effective in treating ICU patients with respiratory failure and is beneficial for improving their lung function,nutritional status,and blood gas parameters,making it worthy of promotion. 展开更多

A statistical formalism overcoming some conceptual and practical difficulties arising in existing two-phase flow (2PHF) mathematical modelling has been applied to propose a model for dilute 2PHF turbulent Hows. Phase interaction terms with a clear physical meaning enter the equations and the formalism provides some guidelines for the avoidance of closure assumptions or the rational approximation of these terms. Continuous phase averaged continuity, momentum, turbulent kinetic energy and turbulence dissipation rate equations have been rigorously and systematically obtained in a single step. These equations display a structure similar to that for single-phase flows. It is also assumed that dispersed phase dynamics is well described by a probability density function (pdf) equation and Eulerian continuity, momentum and fluctuating kinetic energy equations for the dispersed phase are deduced. An extension of the standard k-e turbulence model for the continuous phase is used. A gradient transport model is adopted for the dispersed phase fluctuating fluxes of momentum and kinetic energy at the non-colliding, large inertia limit. This model is then used to predict the behaviour of three axisymmetric turbulent jets of air laden with solid particles varying in size and concentration. Qualitative and quantitative numerical predictions compare reasonably well with the three different sets of experimental results, studying the influence of particle size, loading ratio and flow confinement velocity. 展开更多

[Objectives]To explore the effects of Guizhi Shaoyao Zhimu Decoction on laboratory indicators and immune functions of patients with rheumatoid arthritis.[Methods]A total of 110 patients with rheumatoid arthritis admitted to the Second Department of Orthopedics,the Affiliated Chinese and Western Medicine Hospital of Nanjing,Nanjing University of Chinese Medicine from December 2014 to December 2016 were selected as the research objects and divided into a control group and an observation group using the random number table method,with 55 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Guizhi Shaoyao Zhimu Decoction on the basis of the control group.Both groups were treated for one month.The clinical effects,improvement of clinical symptoms,laboratory indicators and immune function indicator levels between the two groups were compared;the incidence of adverse reactions in the two groups was counted.[Results]The total effective rate of the observation group was 92.73%,higher than that of the control group(78.18%)(P<0.05).Compared with that before treatment,after one month of treatment,the grip strength of the two groups increased,and the observation group was greater than the control group(P<0.05);the morning stiffness time and 20 m walking time of both groups were shortened,and the observation group was shorter than the control group(P<0.05);the number of joint tenderness,joint swelling,ESR,MPV,PDW,serum CRP,plasma IgA,IgG,IgM,RF levels in both groups decreased,and the observation group was lower than the control group(P<0.05);the serum C3 levels in both groups increased,and the observation group was higher than the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group was 0.00%,which was lower than that in the control group(10.91%,P<0.05).[Conclusions]Using Guizhi Shaoyao Zhimu Decoction to treat rheumatoid arthritis can improve the laboratory indicators of patients,and enhance their immune functions,improve the curative effect,and the safety is high. 展开更多

The objective of the present study is to define two important aspects of the computer operating system concerning the number of its vulnerabilities behavior. We identify the Vulnerability Intensity Function (VIF), and the Vulnerability Index Indicator (VII) of a computer operating network. Both of these functions, VIF and VII are entities of the stochastic process that we have identified, which characterizes the probabilistic behavior of the number of vulnerabilities of a computer operating network. The VIF identifies the rate at which the number of vulnerabilities changes with respect to time. The VII is an important index indicator that conveys the following information about the number of vulnerabilities of Desktop Operating Systems: the numbers are increasing, decreasing, or remaining the same at a particular time of interest. This decision type of index indicator is crucial in every strategic planning and decision-making. The proposed VIF and VII illustrate their importance by using real data for Microsoft Windows Operating Systems 10, 8, 7, and Apple MacOS. The results of the actual data attest to the importance of VIF and VII in the cybersecurity problem we are currently facing. 展开更多

Based on the newly-developed element energy projection (EEP) method with optimal super-convergence order for computation of super-convergent results, an improved self-adaptive strategy for one-dimensional finite element method (FEM) is proposed. In the strategy, a posteriori errors are estimated by comparing FEM solutions to EEP super-convergent solutions with optimal order of super-convergence, meshes are refined by using the error-averaging method. Quasi-FEM solutions are used to replace the true FEM solutions in the adaptive process. This strategy has been found to be simple, clear, efficient and reliable. For most problems, only one adaptive step is needed to produce the required FEM solutions which pointwise satisfy the user specified error tolerances in the max-norm. Taking the elliptical ordinary differential equation of the second order as the model problem, this paper describes the fundamental idea, implementation strategy and computational algorithm and representative numerical examples are given to show the effectiveness and reliability of the proposed approach. 展开更多

The use of benthic indicators has increased dramatically during the last decades. The number of articles published on the subject, as well as the number of citations, has been particularly increasing since the early 90’s, notably in relation with the implementation of directives for the management of aquatic/marine ecosystems such as the Water Framework Directive and the Marine Strategy Framework Directive. Current benthic indicators suffer from severe drawbacks and their practical use is still discussed and might have reached a dead end. Indicators based on species composition are not totally satisfactory, mainly because they exhibit a high spatio-temporal variability (e.g. variable at both seasonal and pluri-annual scales) and are user-dependent (e.g. divergent results from US orEuropeexperts.) In turn, modifications of behaviour, metabolism, phenotype or stable isotopes composition in invertebrates usually occur at short time scales, compared to detectable changes in community composition, and makes their use particularly relevant as indicators of perturbation. It is argued in this paper that these functional indicators might be relatively quickly implemented in the intertidal, and represent an effective alternative to current benthic indicators. 展开更多

The proposed ISMC algorithm applied improved self-adaptive crossover and mutation formulae that can provide appropriate crossover operator and mutation operator based on different functions of the objects and the number of iterations. The performance of ISMC was tested by the benchmark functions. The simulation results for residue hydrogenating kinetics model parameter estimation show that the proposed method is superior to the traditional intelligent algorithms in terms of convergence accuracy and stability in solving the complex parameter optimization problems. 展开更多

(1) Evenly distributed initial population is generated. (2) Superior individuals are not broken because of crossover and mutation operation for they are sent to subgeneration directly. (3) High quality im- migrants are introduced according to the condition of the population schema. (4) Crossover and mutation are operated on self-adaptation. Therefore, GSAGA solves the coordination problem between convergence and searching performance. In GSAGA, the searching per- formance and global convergence are greatly improved compared with many existing genetic algorithms. Marchenko imaging obtains the subsurface reflectors using one-way Green’s functions,which are retrieved by solving the Marchenko equation.This method generates an image that is free of spurious artifacts due to internal multiples.The Marchenko imaging method is a target-oriented technique;thus,it can image a user specified area.In the traditional Marchenko method,an accurate velocity model is critical for estimating direct waves from imaging points to the surface.An error in the velocity model results in the inaccurate estimation of direct waves.In turn,this leads to errors in computation of one-way Green’s functions,which then affects the final Marchenko images.To solve this problem,in this paper,we propose a self-adaptive traveltime updating technique based on the principle of equal traveltime to improve the Marchenko imaging method.The proposed method calculates the time shift of direct waves caused by the error in the velocity model,and corrects the wrong direct wave according to the time shift and reconstructs the correct Green’s functions.The proposed method improves the results of imaging using an inaccurate velocity model.By comparing the results from traditional Marchenko and the new method using synthetic data experiments,we demonstrated that the adaptive traveltime updating Marchenko imaging method could restore the image of geological structures to their true positions. 展开更多

The atheroprotective effect of HDL is well-established, the implication being that the higher the plasma levels of HDL,the bigger the benefit to all individuals.Such a supposition has led to the development of "HDL therapy".It has fast become apparent however that assuming therapeutic benefits by merely raising HDL levels is highly simplistic.The most established atheroprotective function of HDL is its role in reverse cholesterol transport(RCT). However other functions of HDL not directly related to RCT may also contribute significantly to its atheioprotective properties.As well,there are many examples where the functionality of HDL changes discordantly to its concentration such that high levels of HDL can be pro-rather than antiatherogenic under certain metabolic conditions.In humans,the ability of HDL to support cholesterol efflux and maintain endothelial function,or antioxidant function can be diminished despite higher HDL levels.Conversely, apoA-IMilano mutation leads to lower HDL levels,but enhances HDL functionality.We and others have recently shown that HDL also has potent anti-inflammatory actions and can also influence the immune system.The inference of these findings are that first,changes in RCT and HDL functionality are as important,if not more so,than its changes in concentration,and second,that these changes are independent of each other.Thus,"HDL therapy" must consider alterations in RCT and HDL functionality as a result of the treatment and the implications that these changes have on the overall atheroprotective effect of the treatment. 展开更多

This study explores the molecular design of sulfur-containing polymers with high refractive indices(RI)and optimized Abbe numbers for advanced optical applications.The high molar refraction and low dispersion of sulfur make it an ideal component for enhancing the optical properties of polymers.Density functional theory(DFT)calculations were employed to predict the RI and Abbe numbers for a range of sulfurbased polymers.To improve the accuracy of the theoretical predictions,a correction function was developed by comparing the calculated values with experimental data.The key polymer families investigated included sulfur-containing polycarbonates,heterocyclic optical resins,and cycloolefins,all modified to balance RI enhancement with dispersion control.The results demonstrate that increasing the sulfur content and introducing specific heterocycles and bridged rings can effectively increase the RI while maintaining desirable Abbe numbers.Polymers incorporating 1,4-dithiane and sulfur-bridged rings exhibit excellent optical clarity and minimal visible light absorption,making them suitable for lens and coating applications.The study also calculated the UV-visible spectra for the most promising polymers,confirming their high transparency.This work establishes a predictive framework for developing high-performance optical polymers and offers a systematic approach for balancing the refractive index and dispersion,thereby providing valuable insights for the design of next-generation optical materials. 展开更多

Objectives To compare respiratory parameters of peripheral blood mononuclear cell mitochondria and iron metabolism indicators in patients with different NYHA functional classes of ischemic heart failure(HF).Methods This single center, prospective, non-blinded study enrolled 20 patients with diagnosed chronic HF of ischemic genesis with reduced and mildly reduced left ventricle ejection fraction. The maximum oxygen consumption at the peak of the exercise test(VO2peak), iron metabolism parameters and respiratory activity of peripheral blood mononuclear cell mitochondria were assessed.Results Among the patients, a half of individuals were diagnosed with iron deficiency. Subgroups of patients with different HF severity did not significant differ in VO2peak(P=0.209), serum iron(P=0.468) and ferritin(P=0.235) levels. But there was a trend in increasing in these parameters with increasing NYHA HF functional class. Respiratory control coefficient(RC) in NADdependent and FAD-dependent mitochondrial oxidation were lower in patients with NYHA HF Ⅲ functional class compared to individuals with NYHA HF I functional class(P=0.028 and P=0.040, respectively). Serum iron(P=0.026), ferritin(P=0.045)levels, transferrin saturation(P=0.006) were negatively correlated with RC in NAD-dependent mitochondrial oxidation.Conclusions In aggravation of ischemic HF NYHA FC, there is a decrease in RC of PBMC mitochondria during the oxidation of NAD-dependent and FAD-dependent substrates. In the whole sample, patients with laboratory-confirmed iron deficiency accounted a half of the total number. Iron metabolism parameters had a paradoxical inverse relationship with the level of RC in PBMC mitochondria of patients with HF. 展开更多

To adapt to national climate change strategies,understanding the thermal vulnerability of urban functional zones(UFZs)is critical for enhancing the livability and sustainable development of cities.We address the limitations of existing thermal vulnerability assessments by incorporating human perception into the analysis.Specifically,we introduce the sky openness index to reflect human perception of the thermal environment and examine variations in thermal vulnerability across different UFZs.Using the Extreme Gradient Boosting(XGBoost)-SHapley Additive exPlanations(SHAP)model,we analyze the contributions of 2D and 3D urban form indicators to thermal vulnerability and link these contributions to the unique needs and vulnerability characteristics of UFZs of Foshan City,China in 2023.The results reveal that:1)high-value heat-fragile areas(1.74–2.00]constitute 10.42%of Foshan City;2)traffic zone and publiccommercial zone exhibit the highest levels of thermal vulnerability;and 3)building height and the normalized building index are the most influential factors,with contributions of|0.06|and|0.03|,respectively.We provide a scientific foundation for developing governance strategies to promote urban resilience. 展开更多

目的分析脑卒中合并肺部感染老年患者病原菌分布及耐药性,并探讨基于护理团队策略下构建的精细化护理模式应用效果。方法选择2022年1月-2025年3月进入本院的92例脑卒中合并肺部感染老年患者作为研究对象,采集所有患者的痰液标本行病原... 目的分析脑卒中合并肺部感染老年患者病原菌分布及耐药性,并探讨基于护理团队策略下构建的精细化护理模式应用效果。方法选择2022年1月-2025年3月进入本院的92例脑卒中合并肺部感染老年患者作为研究对象,采集所有患者的痰液标本行病原菌培养,分析病原菌分布及耐药性情况。采用随机数字表法将患者分为对照组(46例),予以常规抗感染与护理;研究组(46例),在对照组基础上予以护理团队策略下精细化护理模式,两组均护理至出院。对比两组肺部感染指标、肺功能指标[用力肺活量(FVC)、呼气高峰流量(PEFR)、第1s用力呼气容积(FEV_(1))]、动脉血气指标[氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、氧合指数(PaO_(2)/FiO_(2))]、负性情绪[焦虑自评量表(SAS)、抑郁自评量表(SDS)]、护理满意度。结果92例患者共检测出的病原菌包括革兰阴性菌77株,包括肺炎克雷伯菌(27株,29.35%)、鲍曼不动杆菌(18株,19.57%)、铜绿假单胞菌(17株,18.48%)、奇异变形杆菌(6株,6.52%)、流感嗜血杆菌(4株,4.35%)、嗜麦芽窄食单胞菌(3株,3.26%)、罗尔斯通菌(2株,2.17%);革兰阳性菌15株,包括金黄色葡萄球菌(8株,8.70%)、肺炎双球菌(6株,6.52%)、凝固酶阴性葡萄球菌(1株,1.09%)。药敏结果显示,肺炎克雷伯菌对头孢唑啉、头孢呋辛的耐药率均超过50.00%,对于亚胺培南的耐药率超过40.00%;鲍曼不动杆菌对头孢唑啉、头孢呋辛、头孢他啶和头孢吡肟的耐药率均超过55.00%;铜绿假单胞菌对头孢唑啉的耐药率超过50.00%;金黄色葡萄球菌、肺炎双球菌对克林霉素、红霉素、左氧氟沙星的耐药率均为0.00%。研究组肺部感染症状消失时间、体温复常时间、白细胞复常时间、住院时间均短于对照组(P<0.05)。研究组护理后PaO_(2)、PaO_(2)/FiO_(2)高于对照组,PaCO_(2)低于对照组(P<0.05)。研究组护理后FEV_(1)、FVC、PEFR、护理满意度评分均较对照组明显升高(P<0.05)。研究组护理后SAS评分、SDS评分较对照组明显降低(P<0.05)。结论老年脑卒中合并肺部感染患者病原菌以耐药革兰阴性菌为主,临床应参考药敏结果精准用药,开展护理团队策略下精细化护理模式能有效改善血气指标及肺功能,利于缓解患者负性情绪,加速康复进程,患者护理满意度更高。 展开更多

基于指标函数弱面理论的裂缝性白云岩地层井壁稳定性预测

勘探开发实践揭示四川盆地西部地区中三叠统雷口坡组裂缝性白云岩各向异性常导致常规井壁稳定模型预测精度低、井壁失稳频发。为此,通过室内力学实验,系统分析了裂缝倾角与钻井液作用对白云岩力学性质的影响,并在传统弱面理论的基础上... 勘探开发实践揭示四川盆地西部地区中三叠统雷口坡组裂缝性白云岩各向异性常导致常规井壁稳定模型预测精度低、井壁失稳频发。为此,通过室内力学实验,系统分析了裂缝倾角与钻井液作用对白云岩力学性质的影响,并在传统弱面理论的基础上提出了基于指标函数的弱面理论,引入4个各向异性形态参数,使岩石内聚力与内摩擦角随裂缝倾角连续变化,进而将新强度准则与井周应力场耦合,建立了一种能够精确表征岩石复杂力学行为的井壁坍塌压力预测新模型。研究结果表明:①雷口坡组白云岩储层内部发育大量高角度构造裂缝及溶蚀缝洞,这种复杂的裂缝网络结构是该地层宏观力学行为呈现显著各向异性的根本地质成因;②白云岩强度随裂缝倾角呈非对称“V”形分布,最低点位于约60°倾角处,参数ai和bi分别控制强度各向异性的幅值与过渡形态;③实验证实钻井液的侵入与浸泡会提高孔隙压力及弱化岩石结构面,显著劣化了岩石力学特性,钻井液浸泡72 h后,坍塌压力密度上升14.8%。结论认为:①新模型预测值与实际钻井液密度的相对误差仅为0.20%,明显优于Jaeger理论和Mohr-Coulomb准则;②所建立的指标函数弱面理论及耦合流固效应的坍塌压力预测模型,能够更真实地反映裂缝性白云岩的各向异性力学响应,为裂缝性地层井壁稳定性评价提供了可靠的理论依据与工程参考。 展开更多

维奈克拉与阿扎胞苷联用对老年急性髓系白血病患者血液学指标及无进展生存期的影响

目的探讨维奈克拉联合阿扎胞苷治疗老年急性髓系白血病(AML)的效果。方法选取南阳医学高等专科学校第一附属医院2019年5月至2024年7月收治的84例老年AML患者,按随机数字表法分为对照组(n=42,予以阿扎胞苷)与观察组(n=42,予以维奈克拉联... 目的探讨维奈克拉联合阿扎胞苷治疗老年急性髓系白血病(AML)的效果。方法选取南阳医学高等专科学校第一附属医院2019年5月至2024年7月收治的84例老年AML患者,按随机数字表法分为对照组(n=42,予以阿扎胞苷)与观察组(n=42,予以维奈克拉联合阿扎胞苷)。比较两组临床疗效、血液学指标、免疫功能、无进展生存期(PFS)及不良反应情况。结果观察组客观缓解率(ORR)为88.10%,高于对照组的69.05%(P<0.05)。治疗后,观察组血红蛋白(Hb)为(108.43±11.28)g/L,血小板(PLT)为(118.87±9.67)×10^(9)/L,高于对照组的(93.17±9.64)g/L、(106.34±8.16)×10^(9)/L;观察组白细胞计数(WBC)为(6.40±1.39)×10^(9)/L,低于对照组的(9.36±1.94)×10^(9)/L(P<0.001);观察组T淋巴细胞[CD3^(+)(63.58±5.74)%]、CD4^(+)[(44.38±5.46)%]高于对照组[(57.24±5.18)%、(39.56±4.85)%];观察组CD8^(+)[(20.39±3.24)%]低于对照组[(24.16±3.64)%](P<0.001);观察组中位PFS为14.0个月,长于对照组的10.7个月(P<0.001);不良反应发生率组间对比无统计学差异(P>0.05)。结论维奈克拉联合阿扎胞苷治疗老年AML疗效确切,可明显改善患者血液学指标,保护免疫功能,延长PFS,且安全性良好。 展开更多

目的:探究坤泰胶囊联合雌二醇片/雌二醇地屈孕酮片治疗肾虚型卵巢储备功能下降及卵巢早衰患者疗效。方法:纳入2023年6月—2025年6月本院收治的卵巢储备功能下降及卵巢早衰患者136例,随机数字表法分为雌二醇组(n=68,给予雌二醇片雌二醇... 目的:探究坤泰胶囊联合雌二醇片/雌二醇地屈孕酮片治疗肾虚型卵巢储备功能下降及卵巢早衰患者疗效。方法:纳入2023年6月—2025年6月本院收治的卵巢储备功能下降及卵巢早衰患者136例,随机数字表法分为雌二醇组(n=68,给予雌二醇片雌二醇地屈孕酮片治疗)与坤泰胶囊组(n=68,给予坤泰胶囊联合雌二醇片/雌二醇地屈孕酮片治疗)。对比两组临床疗效,治疗前后卵巢情况、性激素水平、血脂水平变化,药物不良反应。结果:治疗后,坤泰胶囊组临床疗效(95.6%)高于雌二醇组(85.3%),两组子宫内膜厚度、卵巢体积、窦卵泡数量均升高,E_(2)、HDL-C水平升高,FSH、LH、LDL-C均下降,且坤泰胶囊组变化幅度大于雌二醇组(均P<0.05);不良反应坤泰胶囊组(11.8%)与雌二醇组(8.8%)无差异(P>0.05)。结论:坤泰胶囊联合雌二醇片/雌二醇地屈孕酮片治疗卵巢早衰,提高临床疗效、卵巢储备功能,改善患者子宫内膜厚度、血清性激素和血脂水平,未增加药物不良反应。 展开更多
