AIM:To study the relationships between amplitude of low-frequency fluctuations(ALFF)changes and clinical ophthalmic parameters in patients with primary open angle glaucoma(POAG)and analyze the diagnostic value of ALFF...AIM:To study the relationships between amplitude of low-frequency fluctuations(ALFF)changes and clinical ophthalmic parameters in patients with primary open angle glaucoma(POAG)and analyze the diagnostic value of ALFF.METHODS:Twenty-four POAG patients and 24 healthy controls(HCs)underwent resting-state functional magnetic resonance imaging(rs-fMRI).Nonparametric rank-sum tests were used to compare the ALFF values in the slow-4 and slow-5 bands,and Spearman or Pearson correlation analysis was used to assess the correlation between ALFF changes and clinical ophthalmic parameters in POAG patients.Receiver operating characteristic(ROC)curves were used to evaluate the diagnostic performance of the ALFF.RESULTS:There were 16 males in POAG patients(median age 48y)and 12 males in HCs(median age 39y).Compared with HCs,POAG patients presented increased or decreased ALFF values in different brain regions,and similar changes were observed in mild POAG patients.The ALFF values were correlated with retinal nerve fiber layer(RNFL)thickness,inner limiting membrane-retinal pigment epithelium thickness changes and the degree of visual field defects.Analysis of the diagnostic value of the ALFF via ROC curves revealed that the right medial frontal gyrus[area under the curve(AUC)=0.9063]and superior frontal gyrus(AUC=0.9097)had better diagnostic value than did the optic disc area(AUC=0.8019),visual field index(VFI%,AUC=0.8988)and macular parameters.CONCLUSION:POAG patients present altered cortical function that is significantly correlated with the optic nerve and retinal thickness and had good diagnostic value,which may reflect the underlying neuropathological mechanism of POAG.展开更多
BACKGROUND Leuconostoc garlicum is commonly found in fermented foods and very few infected patients have been reported,who typically present symptoms such as fever and fatigue.Conventional clinical examinations often ...BACKGROUND Leuconostoc garlicum is commonly found in fermented foods and very few infected patients have been reported,who typically present symptoms such as fever and fatigue.Conventional clinical examinations often struggle to identify this bacterium,and routine anti-infective treatments are generally ineffective.Both diagnostic challenges and therapeutic limitations pose significant difficulties for clinicians.CASE SUMMARY We report a patient ultimately diagnosed with Leuconostoc garlicum infection.The primary manifestations included persistent fever,cough and fatigue.These symptoms lasted for 2 months.He received anti-infective treatment at a community hospital,but this was ineffective.After inquiring about the patient's medical history and conducting a physical examination,the patient underwent laboratory tests.Complete blood count tests revealed that the patient had a high proportion of neutrophils,C-reactive protein level was 235.9 mg/L,erythrocyte sedimentation rate was 67 mm/h,respiratory pathogen testing was negative,and he was then thought to have an infectious disease.However,conventional anti-infective treatments were ineffective.After excluding infectious neurological diseases,urologic diseases and digestive problems,we ultimately focused our attention on the lungs.A lung computed tomography scan indicated pulmonary inflammation.Bronchoalveolar lavage fluid for next-generation sequencing suggested lung infection with Leuconostoc garlicum.The patient's symptoms gradually improved following treatment with piperacillin tazobactam and linezolid.During the follow-up period,the patient's temperature remained normal.CONCLUSION For patients with suspected bacterial infection and experiencing fever,conventional anti-infective treatment can be ineffective in controlling their symptoms,and an infection due to rare bacteria or drug-resistant bacteria should be considered.Next-generation sequencing enables rapid and precise identification of infection-related pathogens in febrile patients.展开更多

Pulmonary rehabilitation (PR) is an established intervention for the management of patients with COPD. Exercise training is an important part of PR, and its effectiveness in patients with COPD is well established. However, alternative methods of PR training such as Daoyin have not been appropriately studied. Hence, alternative forms of exercise training that require less exercise equipment and no specific training place should be evaluated. This paper describes the study protocol of a clinical trial that aims to determine if pulmonary Daoyin training will improve the exercise capacity and psychosocial function of patients with COPD in China. METHODS AND DESIGN: A multicenter, randomized, controlled trial will be conducted. A total of 464 patients meeting the inclusion criteria will be enrolled into this study with 232 patients in each of the trial group and the control group. Based on patient education, patients in the trial group will receive pulmonary Daoyin and continue with their usual therapy for three months. In the control group, patients will continue with their usual therapy. The primary outcome measures are exercise capacity assessed by the six-minute walking distance test and lung function. Secondary outcomes include dyspnea and quality of life. Measurements will be taken at baseline (month 0) and after the study period (month 3). DISCUSSION: It is hypothesized that pulmonary Daoyin will have beneficial effects in improving exercise capacity and psychosocial function of patients with stable COPD, and will provide an alternative form of exercise training that is accessible for the large number of people with COPD. Currently, few treatments are available for IPF. Clinical practice of traditional Chinese medicine(TCM), using a syndrome differentiation approach, offers some treatment success in IPF. However, there is no sufficient evidence-based study of the role of TCM in IPF management to make strong conclusions. This study evaluates the efficacy and safety of TCM in the treatment of IPF.Methods and design: A multicenter, exploratory, randomized, double-blind and placebo-controlled trial is planned. A total of 80 patients will be enrolled in the study, which will include 26 weeks of treatment.Participants will be randomly assigned into TCM group or control group in a 1:1 ratio. The TCM group will be given TCM granules based on syndrome differentiation. Formulae include Bao-fei Hua-xian granule for lung qi deficiency, Jin-shui Huan-xian granule for lung-kidney qi deficiency and Yang-qing Kang-xian granule for yin deficiency and inner heat. The control group will be given a corresponding TCM granule placebo. The efficacy and safety of interventions will be evaluated by the outcome variables,including frequencies of acute exacerbations, pulmonary function, clinical symptoms, dyspnea, healthrelated quality of life(HRQo L), 6-minute walk distance and safety indicators.Discussion: It is hypothesized that TCM will decrease the frequency of adverse events, improve pulmonary function and HRQo L, based on our clinical experience. The present study aimed...Background: Lupus nephritis (LN) is classified by renal biopsy into proliferative and nonproliferative forms, with distinct prognoses, but renal biopsy is not available for every LN patient. The present study aimed to establish an alternate tool by building a predictive model to evaluate the probability of proliferative LN. Methods: In this retrospective cohort with biopsy-proven LN, 382 patients in development cohort, 193 in internal validation cohort, and 164 newly diagnosed patients in external validation cohort were selected. Logistic regression model was established, and the concordance statistics (C-statistics), Akaike information criterion (AIC), integrated discrimination improvement, Hosmer-Lemeshow test, and net reclassification improvement were calculated to evaluate the performance and validation of models. Results: The prevalence of proliferative LN was 77.7% in the whole cohort. A model, including age, gender, systolic blood pressure, hemoglobin, proteinuria, hematuria, and serum C3, performed well on good-of-fit and discrimination in the development chohort to predict the risk of proliferative LN (291 for AIC and 0.84 for C-statistics). In the internal and external validation cohorts, this model showed good capability for discrimination and calibration (0.84 and 0.82 for C-statistics, and 0.99 and 0.75 for P values, respectively). Conclusion: This study developed and validated a model including demographic and clinical indices to evaluate the probability of presenting proliferative LN to guide therapeutic decisions and outcomes.展开更多

Magnetic impurities in superconductors are of increasing interest due to emergent Yu-Shiba-Rusinov(YSR)states and Majorana zero modes for fault-tolerant quantum computation.However,a direct relationship between the YS...Magnetic impurities in superconductors are of increasing interest due to emergent Yu-Shiba-Rusinov(YSR)states and Majorana zero modes for fault-tolerant quantum computation.However,a direct relationship between the YSR multiple states and magnetic anisotropy splitting of quantum impurity spins remains poorly characterized.By using scanning tunneling microscopy,we systematically resolve individual transition-metal(Fe,Cr,and Ni)impurities induced YSR multiplets as well as their Zeeman effects in the K_(3)C_(60)superconductor.The YSR multiplets show identical d orbital-like wave functions that are symmetry-mismatched to the threefold K_(3)C_(60)(111)host surface,breaking point-group symmetries of the spatial distribution of YSR bound states in real space.Remarkably,we identify an unprecedented fermion-parity-preserving quantum phase transition between ground states with opposite signs of the uniaxial magnetic anisotropy that can be manipulated by an external magnetic field.These findings can be readily understood in terms of anisotropy splitting of quantum impurity spins,and thus elucidate the intricate interplay between the magnetic anisotropy and YSR multiplets.展开更多

文摘To the Editor:Serum alkaline phosphatase (ALP) is an important bone turnover marker in chronic kidney disease (CKD) patients, with studies indicating that higher serum ALP levels are associated with increased mortality in hemodialysis (HD) patients.[1] Our previous study also showed that higher serum ALP levels were independently associated with all-cause and cardiovascular mortality in peritoneal dialysis (PD) patients.

文摘Magnetic impurities in superconductors are of increasing interest due to emergent Yu-Shiba-Rusinov(YSR)states and Majorana zero modes for fault-tolerant quantum computation.However,a direct relationship between the YSR multiple states and magnetic anisotropy splitting of quantum impurity spins remains poorly characterized.By using scanning tunneling microscopy,we systematically resolve individual transition-metal(Fe,Cr,and Ni)impurities induced YSR multiplets as well as their Zeeman effects in the K_(3)C_(60)superconductor.The YSR multiplets show identical d orbital-like wave functions that are symmetry-mismatched to the threefold K_(3)C_(60)(111)host surface,breaking point-group symmetries of the spatial distribution of YSR bound states in real space.Remarkably,we identify an unprecedented fermion-parity-preserving quantum phase transition between ground states with opposite signs of the uniaxial magnetic anisotropy that can be manipulated by an external magnetic field.These findings can be readily understood in terms of anisotropy splitting of quantum impurity spins,and thus elucidate the intricate interplay between the magnetic anisotropy and YSR multiplets.
