摘要
胰瘘和胆瘘是胰十二指肠切除术后的近期常见并发症,发生于术后1个月内。主胰管的支撑引流可降低胰内压力防止胰液渗出,有利于胰肠吻合口愈合,胆管的支撑引流可降低肝内胆管压力防止胆液渗出,有利于胆肠吻合口愈合。本文报告一例病人的腹腔镜胰十二指肠切除术中内镜鼻胆胰管引流术,探讨术中内镜鼻胆胰管引流手术方法的可行性。
Pancreatic fistula and biliary fistula are the most common recent complications after pancreaticoduodenectomy and occur within 1 month after surgery. The supportive drainage of the main pancreatic duct can reduce the internal pressure of the pancreas and prevent pancreatic fluid from exudation, which is conducive to the anastomosis of the pancreas and intestine. The supportive drainage of bile duct can reduce the pressure of intrahepatic bile duct and prevent the sapporous exudation, which is beneficial to the healing of biliary anastomosis. This paper reports a case of endoscopic nasobiliopancreatic duct drainage during laparoscopic pancreaticoduodenectomy, and discusses the feasibility of endoscopic nasobiliopancreatic duct drainage during laparoscopic pancreaticoduodenectomy.
