摘要
目的 分析和探讨应用标准外伤大骨瓣开颅治疗重型颅脑损伤。方法 回顾性总结一组 36例重型颅脑损伤应用标准外伤大骨瓣开颅疗效。结果 本组病例存活 2 5例 (6 9 4 % ) ,其中GCS3～ 5分存活 5例 (4 1 7% ) ,GCS6～ 8分存活 2 0例(83 3% )。随访半年 ,按GOS :治愈 16例 ,中残 5例 ,重残 2例 ,植物生存 2例。死亡 11例 (30 6 % )。结论 应用标准外伤大骨瓣开颅暴露范围广 ,能有效清除幕上颅内血肿、坏死组织及降低颅内压 ,提高重型颅脑损伤救治成功率。
Objective To summarize the surgical treatment of severe traumatic head injury by large decompression craniotomies.Methods The surgical treatment by large decompression craniotomies of the thirty-six severe traumatic head injured patients was analyzed retrospectively.Results 25 cases (69.4%) survived in 36 cases. 5 cases (41.7%) survived in 12 cases with a score of 3～5 on GCS and 20 cases (83.3%) survived in 24 cases with a score of 6～8 on GCS . All the patients were followed up over 6 months , 16 cases survived well , 5 cases suffered from moderate disability ,2 cases with severe deficits and 2 cases vegetative according to GOS. The other 11 cases died (30.6%) after therapy.Conclusion The application of standard large decompression craniotomies with a good exposure could evacuate intracranial hematoma above tentorium cerebelli and dead brain tissue effectively , decrease intracranial pressure ,and increase the cure ratee sever of traumatic head injury patients.
