摘要
目的探讨改良式前路减压骨笼植入术的临床应用价值.方法从1999年6月～2003年6月,共行颈椎前路手术应用AIFC(allograft interbody fusion cage,AIFC)融合256例,其中实验组74例,采用改良式前路减压骨笼植入术,对照组182例,采用传统的环锯减压骨笼植入法;术后比较两组AIFC的融合率及椎间隙的高度变化.结果手术临床疗效评判优良236例,其中实验组脊髓型颈椎病患者优良率较对照组有明显差异;实验组病例有62例椎间隙高度轻度增高,对照组病例椎间隙无明显变化,两组差异明显,有统计学意义.AIFC与椎体融合良好,未发现排斥反应.实验组有5例骨笼崩裂,对照组有4例骨笼崩裂,P＞0.1,两组无显著性差异结论改良式前路减压骨笼植入术能有效地恢复和维持椎间隙高度,对脊髓型颈椎病具有良好的治疗效果.
Objective:To discuss the clinical value of improved anterior cervical spine decompression and interbody fusion.Methods:From June1999to June2003,256patients accepted anterior cervical spine decompression and interbody fusion with AIFC(allograft interbody fusion cage).In test group,74cases of256accepted improved anterior cervical spine decompression and interbody fusion while in control group,182ac-cepted traditional anterior cervical spine trepan decompression and interbody fusion;then the rate of AIFC fusion,maintenance of intervertebral height were compared between the two groups.Results:Effect in236of256cases was good.The rate of good effect in cervical spondylosis of spinal cord type is significant differ-ence in two groups.Intervertebral height in62cases of test group lightly increased.While in control group,intervertebral height didn't change.There was significant difference between the two groups.There was no re-jection between AIFC and patients.AIFC in5cases of test group broke,while in4cases of control group broke,P>0.1,and there was no significant difference between the two groups.Conclusions:Improved anterior cervical spine decompression and interbody fusion can recover and maintain intervertebral height,it is effec-tive in cervical spondylosis of spinal cord type.
