摘要
目的 探讨缝匠肌蒂骨瓣植骨治疗股骨骨不连手术效果与手术指征选择的关系。方法 游离缝匠肌长约 15cm ,骨凿凿取髂骨瓣约 5cm× 4cm大小。将髂骨瓣及缝匠肌呈游离状后向下翻转 ,于股外侧肌肌腹下打一隧道 ,将缝匠肌肌蒂髂骨瓣从股外侧肌肌腹下隧道穿出 ,髂骨修整碎骨块嵌入骨折间隙或内后侧 ,带肌蒂骨瓣植于骨折断端前外侧。结果 11例均作随访 ,骨折对位对线良好 ,骨折断端骨性愈合 ,达到了治疗目的。结论 缝匠肌蒂骨瓣主要是改善了股骨骨折断端血液循环 ,扩大骨折断端接触面 ,同时又起到了常规植骨的作用 ,诱发成骨生长 ,促进骨折愈合 ,提高了手术成功率。
Objective To investigate the clinical effects treated the non-union of the femur fracture by the bone transplantation of the iliac steocutaneous graft with the sartorius muscle.Methods 11 patients affected by the non-union of femur fracture were treated though the bone transplantation of the iliac block graft with the sartorius muscle. The average size of iliac block graft was 4.0cm×5.5cm.The average length of free sartorius muscle was 15cm.Results All patients were followed up after operation. All the fractures were united and the iliac block grafts were healed with two ends of fracture simutaneously.Conclusion The iliac block graft with sartorious muscle is of sufficient blood supply and survived osteocytes. Using it to treat the non- union of the femur fracture can improve blood supply of two fracture ends, enlarge fracture terminate touching and accelerate the fracture to heal.
