摘要
本文报道采用切除有病变的肝叶,特别是左外肝叶以治疗肝内胆管结石和狭窄。本组32例经肝叶、段切除及肝断面胆管剖开,彻底解除梗阻和狭窄、取净结石,建立通畅的引流,Roux-en-Y或盆式胆肠吻合术;术后通过T管造影、B超和临床随访,其中3例因肝硬化术后肝衰死亡外,其余29例中属优者26人,良者2人,差者1人,总优良率占96.55％。
Stones in intrahepatic bile duct and bile duct stenos is in liver have become a clinical problem in surgery. The series of pathological changes of liver cells and bile duct in liver are not reversed.So we consider it is important to resect the diseased liver lobes,especially stones in bile duct and bile duct stenosis located at lobes of liver ,left. In our 3.2 patients ,after resecting the liver lobes or segment ,openning the long way of residual bile duct through the planes of parenchymal transection,we removed all of the obstruction or stenosis and stones,choledoenterostomy was made with a wide anastomosis by Roux-en-Y. A long time followup after operation,out of 29 cas-es (92. 6%) examined by cholangiography,B typed ultrosound or clinical study,only 3 cases died by liver failure. According to the criterion of Xia Liangfang made in 1988,the result showed:26 cases (89. 66%) were excellent,2 cases (6. 89%) were good,only one case (3. 45%) was bad. 96. 55% of all patients showed good or very good.
