摘要
【目的】探讨腹水超滤回输腹腔后腔内灌注榄香烯乳治疗癌性腹水的疗效。【方法】将 72例恶性肿瘤并发大量癌性腹水的患者随机分为治疗组和对照组 ,各 36例。治疗组用AR -Ⅰ型自体腹水超滤回输治疗仪将腹水超滤回输腹腔后腔内灌注榄香烯乳 ,对照组于反复常规腹腔穿刺放液后腹腔内灌注榄香烯乳治疗 ,比较两组疗效 ,及治疗前后腹水蛋白和血浆白蛋白的含量、尿量和Karnofsky评分变化 ,并观察毒副反应。【结果】治疗组完全缓解 (CR) 9例 ,部分缓解 (PR) 2 2例 ,无效 (NC) 5例 ,有效率为 86 1% ;对照组CR4例 ,PR2 0例 ,NC12例 ,有效率为 6 6 7% ,两组疗效比较有显著性差异 (P<0 0 5 )。治疗组治疗后腹水蛋白、血浆白蛋白均有明显升高 ,尿量增加 ,Karnofsky标准评分分值明显提高 ,与对照组比较均有显著性差异 (P <0 0 5或P <0 0 1)。【结论】腹水超滤回输腹腔后腔内灌注榄香烯乳治疗癌性腹水有较好的疗效 ,能明显提高患者的生活质量 ,无明显毒副反应 ,值得临床推广应用。
To investigate the therapeutic effect of ascitic fluid ultrafiltration refusion and perfusion of β-elemene milk into abdominal cavity on carcinomatous ascites. Seventy-two cases of malignant tumor complicated with carcinomatous ascites were randomized to two groups: group A treated with perfusion of β-elemene milk into abdominal cavity after ascitic fluid ultrafiltration refusion and group B with perfusion of β-elemene milk into abdominal cavity after letting-out of ascitic fluid by regular and repeated abdominal paracentesis. Therapeutic effect was compared and ascitic protein (AP) , plasma albumin(PA) content, urine volume and Karnofsky scores were observed before and after treatment as well as the toxic and side effects.In group A, 9 cases were completely relieved (CR) , 22 partially relieved (PR) , 12 not changed (NC) and the effective rate was 86.1%;in group B,4 were CR, 20 PR, 12 NC and the effective rate was 66.7% respectively, the differences being significant (P<0.05) . After treatment, ascitic protein, plasma albumin content, urine volume and Karnofsky scores were increased in group A as compared with those before treatment and in group B (P<0.05 or P<0.01).[Conclusion]Ascitic fluid ultrafiltration refusion and perfusion of β-elemene milk into abdominal cavity have good effect on carcinomatous ascites, which are capable of improving life quality and have no obvious toxic and side effects.
