摘要
目的 :探讨氨茶碱、可拉明、洛贝林、纳洛酮联合治疗慢性阻塞性肺疾病 (COPD)合并肺性脑病的疗效、安全性及应用的合理性。方法 :对 COPD合并肺性脑病的患者 ,随机分为使用可拉明治疗的 A组 ,使用纳洛酮治疗的 B组及联合使用氨茶碱、可拉明、洛贝林、纳洛酮治疗的 C组 ,并对 3组治疗前后神经精神症状、心率、呼吸频率、血气分析等资料进行对比分析。结果 :A组治疗后神经精神症状及 p H、Pa O2 、Pa CO2 与治疗前相比较稍有改善 ,但变化不显著 (P>0 .0 5 ) ,Pa CO2 仍维持在较高水平 ;B组治疗后神经精神症状消失 ,与 A组及治疗前相比较 ,治疗后其 p H、Pa O2 及 Pa-CO2 均有明显改善 (P<0 .0 5 ) ,但 Pa CO2 维持在较正常值偏高的水平 ;C组治疗后 ,不仅神经精神症状迅速消失 ,而且与 A、B二组及治疗前相比较 ,其 p H、Pa O2 及 Pa CO2 均有极显著改善 (P<0 .0 1) ,有 85 .4 %患者 p H、Pa O2 达正常值水平。结论 :氨茶碱、可拉明、洛贝林、纳洛酮联合治疗 COPD合并肺性脑病 ,能迅速解除通气功能障碍 ,从而迅速扭转病情恶化的趋势 ,改善预后 ,获得较好的临床效果 ,是肺性脑病患者安全、合理的治疗方案之一。
Objective:To explore the efficacy and safety of the combination treatment of ami nophylline,nikethamide,lobeline and naloxone in chronic obstruction pulmonary di sease(COPD) complicated with pulmonary cerebropathy.Methods:Cases of COPD compli cated with pulmonary cerebropathy were divided into three groups.Group A was giv en nikethamide,group B was given naloxone and group C was given the combination of aminophglline,nikethamide,lobeline and naloxone.The nervous and mental situat ion,heart rate,respiratory frequen cy and blood gas were recorded and analyzed before and after the treatment.Resul ts:The nervous and mental symptoms,pH,PaO 2 and PaCO 2 improved slightly (>0.05) in group A.In group B,the nervous and mental sympto ms disappeared and pH,PaO 2 and PaCO 2 improved significantly (P>0.05).In group C,not only the disappear of symptoms,pH,PaO 2 and Pa CO 2 improved remarkably as compared with those in group A and B (P <0.01).pH and PaO 2 reached the normal value in 85.4% of cases.Con clusion :Treating COPD complicated with pulmonary cerebropathy with the combination of a minop hylline,nikethamide,lobeline and naloxone can reliev the respiratory obstruction rapidly,change the worse trend of disease and improve the prognosis.
