摘要
目的 评价大剂量丙种球蛋白联合地塞米松、单独大剂量地塞米松及单独大剂量丙种球蛋白 3种方法治疗重度 ITP的疗效。 方法 对入院时 PLT≤ 2 5× 1 0 9/ L,出血倾向明显的 45例患儿随机分成 3个组 ,每组 1 5例。联合组 :静滴丙种球蛋白 1 .0 g·kg- 1·d- 1 ,连用 2天 ,同时静滴地塞米松 1 .5 mg·kg- 1· d- 1 ,连用 7天 ,第 7天起口服强的松 2 mg·kg- 1·d- 1 ;地塞米松组 :静滴地塞米松 1 .5 mg· kg- 1·d- 1 ,连用 7天 ,第 7天起口服强的松 2 mg· kg- 1 · d- 1 ;丙种球蛋白组 :静滴丙种球蛋白 1 .0 g· kg- 1 · d- 1 ,连用 2天。连续监测治疗效果。 结果 联合组、丙种球蛋白组、地塞米松组使 PLT≥ 5 0× 1 0 9/ L时间分别为 ( 2 .0± 1 .1 )天 ,( 2 .8± 1 .2 )天 ,( 3 .9± 1 .3 )天 ;≥ 1 0 0× 1 0 9/ L时间分别为 ( 4 .0± 1 .4)天 ,( 5 .2±1 .5 )天 ,( 6.9± 1 .7)天 ;PLT的峰值 (× 1 0 9/ L)分别为 ( 3 68± 1 3 1 ) ,( 3 1 7± 1 2 1 ) ,( 2 1 3± 76)。 结论 大剂量丙种球蛋白联合地塞米松静滴治疗重度 ITP比单纯用其中任一种药物升高血小板的作用更为显著 。
Objective To evaluate the effectiveness of high-dose Immunoglobulin combine with Dexamethasone,high-dose Dexamethasoneand high-dose Immunoglobulin in Severe Idiopathic Thrombocytopenic Purpura(ITP). Methods Forty-five patients hospitalized with PLT≤25×10 9/L and phanero-tendency of hemorrhage were randomly divided into three groups (every group n=15).Group of combine:intravenousdripImmunoglobulin 1.0mg·kg -1·d -1 for two days,and intravenousdripDexamethasone 1.5mg·kg -1·d -1 for seven days inthe same times,and after seven days orally prednisolone 2.0mg·kg -1·d -1.Group of Dexamethasone:intravenousdrip Dexamethasone1.5mg·kg -1·d -1 for seven days, after seven days orally prednisolone 2.0mg·kg -1·d -1; Group of Immunoglobulin 1.0g·kg -1·d -1 for two days.In these days,continuous to monitor therapeutic effects. Results In the group ofcombine and group of immunoglobulin and group of Dexamethasone,the times of PLT ≥50×10 9/L were separated for (2.0±1.1) days and (2.8±1.2) days and (3.9±1.3) days; the times of PLT≥ 100×10 9/L wereseparated for (4.0±1.4) days and (5.2±1.5) days and (6.9±1.7)days;PLT-peak values(×10 9/L) were separated for (368±131) and (317±121)and (213±76). Conclusions Intravenousdrip high-dose Immunoglobulincombined Dexamethasone is more rapidly and effectively than anymedications in increasing the platelet count in severe ITP.
