摘要
目的 观察光学性及治疗性穿透性角膜移植术的疗效及并发症。方法 对分别用新鲜角膜和甘油保存角膜做穿透性角膜移植的 180例 ( 180眼 )进行回顾性总结。结果 用新鲜角膜行光学性角膜移植 76例 ,植片透明者 65例 ,占 85 5 %;用甘油保存角膜行治疗性角膜移植 10 4例 ,植片全部浑浊。术后主要并发症 :免疫排斥反应前组 8例 ( 10 5 %) ,后组 48例( 4 6 2 %) ;继发性青光眼前组 3例 ( 3 9%) ,后组 2 0例 ( 19 2 %)。结论 光学性穿透移植可使角膜盲患者复明 ,但防治并发症是成功的关键。治疗性穿透移植可有效去除病灶 ,控制原发病 ,为二期光学性角膜移植创造条件。
Objective To observe the effects and complications of penetrating keratoplasty (PKP). Methods Retrospective analysis on 180 cases(180 eyes) of PKP were treated by fresh donor corneas and pure glycerine preserved corneas. Results Optical PKP was performed on 76cases using fresh donor corneas, the rate of clarity was 85.5%(65cases) and the percentage of visual acuity less than 0.04 was 40.8%(31 cases), 0.05～0.2 was 42.1%(32cases), better or equal to 0.3 was 17.1%(13 cases). The main complications were rejection(10.5%) on the graft and secondary glaucoma(3.9% ). Therapeutic PKP was performed on 104 cases using donor corneas preserved in pure glycerine, the diseases was controlled and the grafts lived but became opacity. The main complications were graft rejection(46.2%),secondary glaucoma (19.2%) and recurrence of diseases(8.7%). Conclusions Optical PKP might reserve sight of blindness from corneal diseases, but the prevention and treatment of complication were important for successful surgery. Therapeutic PKP can control infection effectively and provide support for optical PKs later.
