血管性假血友病因子和细胞间黏附分子-1在急性呼吸窘迫综合征中的动态变化及早期诊断的意义 被引量:5

Variations and early prediction of von willebrand factor and soluble intercellular adhesion molecule-1 in acute respiratory distress syndrome

摘要 目的 检测血管性假血友病因子、可溶性细胞间黏附分子 1浓度变化 ,以研究其在ARDS早期诊断中的意义。方法 以ELISA法检测非ARDS组和ARDS组于 0、 2、 3、 5和 7d血浆vWF和sICAM 1浓度。结果 ARDS组vWF和sICAM 1浓度高于非ARDS组 ,前四个时点的中性粒细胞活化计数ARDS组高于非ARDS组。血浆vWF和sICAM 1水平具有相关性。结论 血浆vWF和sICAM 1浓度及中性粒细胞活化计数在ARDS的早期即升高 ,对ARDS早期诊断有预警作用。 Objective To measure the plasma levels of vWF and sICAM-1 in patients at risk for the development of ARDS,and to evaluate their significance in early diagnosis of ARDS.Methods Plasma vWF and sICAM-1 were both measured with ELISA in patients without ARDS group and with ARDS group on admission and at day 2,3,5,7. Results A statistically significant difference was found in the mean vWF level for ARDS and non-ARDS patients at the first three time points. And the sICAM-1 levels between the two groups at the first four time points also showed a statistical significant difference.The counts of activated neutrophil at the first four time points of ARDS group were significantly greaterthan those in non-ARDS group. There was a positive correlation between plasma levels of vWF and sICAM-1.Conclusion Plasma levels of vWF and slCAM-1 and the counts of activated neutrophil were elevated in early ARDS. These observations suggest that there are injury of endothelium andneutrophil activation in early ARDS.
