摘要
目的 探讨保留颈动脉手术治疗累及颈动脉的口腔颌面部恶性肿瘤可行性。方法 对 2 1例累及颈动脉的口腔颌面部恶性肿瘤实施手术 ,术中剥离颈动脉周膜或外膜 ,保留颈动脉切除肿瘤。结果 2 1例无术中死亡。 2 1例均获随访 ,术后复发 1 0例 ,其中颈部复发 7例 (6个月内复发 3例 ,9、1 4、1 6、1 8个月复发各 1例 ) ,均死亡。口底、腮腺、颌下腺术后复发各 1例。术后无肿瘤复发者 1 1例。术后 1年以上健在者 1 8例 ,3年以上 9例 ,5年以上 7例 ,1、3、5年生存率分别为 85 7%、4 2 9%和 33 3%。
Objective To study the feasibility of surgical therapy of oral and maxillofacial malignancy invading the carotid artery without resection of the carotid artery.Method Selected 21 patients of malignancy invasion of the carotid artery were treated with retaining the carotid artery by dividing the periarterial fascia or adventitia.Result Twenty one patients have been followed up.Seven of the 21 patients recurred in neck.Three patients receurred in floor of mouth,parotid and submandibular gland.Survival was>12 months in 18/21(85.7%)patients.Nine patients(42.9%) survived over three years and seven(33.3%) patients over five years.Conclusion It is feasible to resection the tumor with retaining carotid artery.
