丝裂霉素-C眼内应用抑制后囊膜混浊的实验研究 被引量:4

摘要 目的 通过动物实验 ,研究眼内应用浓度为 0 2mg/ml的丝裂霉素 -C (MMC)对于术后后囊膜混浊的抑制作用。方法 将 3 2只实验动物随机分为A ,B ,C ,D四组。所有的实验动物均进行晶状体超声乳化摘除术。A组为对照组 ,B组术中应用 0 2mg/ml的MMC溶液作水分离 ,C组术中将混有 0 2mg/mlMMC的粘弹剂注入囊袋中 ,D组将MMC溶于配好的灌注液 ,术中应用。术后进行术眼裂隙灯检查、房水免疫球蛋白测定及角膜内皮细胞分析 ,术后 1月进行术眼后囊照相。结果 术后 1天和 1周 ,D组角膜水肿的例数及持续时间均明显高于其他三组。角膜内皮细胞分析显示D组的角膜内皮数量明显低于其余三组。术后 1天、 1周 1月时D组房水中IgG含量明显高于同时期的其余三组。其余三组无显著性差异。术后 1月术眼后囊照相可见A组中央后囊明显混浊 ,B、C、D组中央后囊清亮。结论 实验显示眼内应用 0 2mg/ml的MMC明显抑制了后囊膜混浊的发展。尽管眼内应用 0 2mg/ml的MMC可以导致眼内组织毒性反应 ,但通过用药方法的改进和保护措施的加强是可以有效避免或减轻的。 Objective We investigate whether mitomycin C(0 2mg/ml)can inhibit posterior capsule opacification without causing ocular toxicity.Methods Thirty two New Zealand rabbits were randomized into four groups.Group A:Received phacoemulsification without applying MMC.Group B:Received hydrodissection using a solution containing MMC(0 2mg/ml).Group C:Received MMC dissolved in sodium hyaluronate(0 2mg/ml)which was injected into the empty capsular bag after phacoemulsification.Group D:Received MMC dissolved in irrigating solution (0 2mg/ml)which was used for irrigation.Al groups were checked with slit lamp and we detected the IgG in aqueous humor 1 day,1 week,1 month after surgery.The corneal endothelium of all groups was checked 1 week and 1 months after surgery.Results The duration of corneal edema in group D were markedly longer than in the other there groups 1 day and 1 month after surgery.The enodthelium counting of group D was markedly lower than of the other three groups.The quantity of Ig G of aqueous humor in group D was significantly higher than in the other groups 1 day,1 week and 1 month after surgery.The photomicrograph indicated that there was a large amount of regenerated cortical material in the peripheral area and in the center of of lens capsule in group A.In group B,C and D,a small quantity of cortical material can be seen in the peripheral area of lens capsule.There were no fibroblasts or Elschnig pearls in the center of posterior capsule.Conclusions In our experiment the application of MMC(0 2mg/ml)in phacoemulification can effectively reduce PCO.Although the intraocular application of MMC(0 2mg/ml)can induce drug toxicity to intraocular tissues.Some drug application methods can reduce or prevented the drug toxicity to intraocular tissues.
