摘要
目的 对 30例行永久性耻骨上膀胱造瘘术门诊定期更换导管的患者进行膀胱内定植菌培养检测 ,为药物治疗及预防导管伴随性尿路感染提供依据。方法 在导管与集尿器衔接处用碘伏棉签局部消毒后用 5 ml空针抽取拔管前、重新置管后的尿样标本各 5 ml,置于无菌容器中立即送检 ,操作过程严格执行无菌操作原则。结果30例患者膀胱内均有不同类别的定植菌 ,置管后采取常规饮水及用 0 .9%生理盐水密闭式冲洗 ,采集尿液样本细菌培养检测 ,统计学处理χ2 检验细菌检出阳性率有明显统计学差异。结论 永久性耻骨上膀胱造瘘术的患者 ,是潜在的医院感染因素 ,操作得当、用药合理、防护到位可以减少或避免全身感染的发生。
OBJECTIVE To test bacteriopexia in patients after permanent epicystostomy in order to provide the bases for medical treatment and prevention against infection from urinary organs accompanied with the ducts. METHODS Thirty patients who accepted the operation of permanent epicystostomy and regularly change the ducts received testing of bacteriopexia cultivation from their bladders. Five milliliters urine samples were respectively taken with 5 ml empty injectors before pulling out the ducts and after fixing them again. After locally sterilizing the point that connected ducts and urine collector with iodic cotton stick,the samples were put in the aseptic containers and sent to test immediately. The whole process was strictly carried out according to the principle of aseptic manipulation. RESULTS Thirty patients had various sorts of bacteriopexia in their bladders. After fixing ducts,the regular drinking water and 0.9% normal saline were used to air-tightly wash. Then urine sample was taken to test bacterial cultivation. There was significantly statistical difference in the aspect of positive rate after testing the bacteria with statistical operating χ 2. CONCLUSIONS The patients after permanent epicystostomy were potential infectious factors in hospitals. They are potential dangers as well to their own health. Proper manipulation,treatment and protection can reduce or avoid the systemic infection.
