摘要
目的分析极外侧型腰椎间盘突出症(FLLDH)的临床特点,探讨合理的术式。方法回顾总结18例FLLDH的临床资料。分别应用经椎板间入路(术式1)、经峡部外缘入路(术式2)和经横突间入路(术式3)三种术式单独或联合进行治疗。结果经平均1年2个月随访,依据Macnab疗效标准,优12例,良5例,可1例,差无,优良率为94.4%。结论本症多具有较高神经节段受损的症状和体征。结合临床的椎间孔区高质量CT检查是诊断FLLDH的关键。本组所选术式既能彻底摘除致压髓核组织,又最大限度地保留了脊柱的完整性。
Objective To investigate the characteristics of far lateral lumbar disc herniation and evaluate the rational operative techniques. Methods A retrospective evaluate on study was made for 18 cases of FLLDH. Three various operative approachs were used to treat the disorder in different styles,including the translaminar osteotomy(operative technique 1),the group approached from the outer edge of par interarticularis(operative technique 2),the transverse osteotomy(operative technique 3). Results All cases were followed up for 14 months on average,according to the Macnab standard,the excellent and good rate was 94.4% with excellent 12 cases,good 5 cases,acceptable 1 case and no poor case. Conclusion The symptoms and signs in physical examinations of the disorder mainly result from injury of upper nerve segments.The key point of FLLDH diagnosis is CT scanning of high quality in the area of intervertebral foramen and accorded clinical manifestations.All operative techniques in the group not only remove nucleus pulposus completely,but slso thoroughly decompression of lateral recessus and maintain the integrity of spine.
