摘要
目的 探讨应用超声造影剂Levovist(利声显 )对颅内动脉瘤能量多普勒血流显像的增强效果及其临床应用价值。方法 对临床疑为动脉瘤的患者 ,经常规彩色多普勒检查筛检出 10例诊断不确切、显影不佳的患者行造影增强 ,使用Levovist ,浓度 2 0 0～ 40 0mg/ml,经肘正中静脉持续推注 ,速度 2ml/min ,观察造影后不同时间病灶及其他血管能量多普勒血流显像增强效果 ,记录各血流参数。所有病例与数字减影血管造影术双盲对照。结果 受检者注射造影剂后 10～ 2 0s开始显影并很快出现高峰 ,显影增强持续时间为 3 .5～ 9.0min ,多普勒显像明显增强 ;造影后 9例患者均显示动脉壁上“囊袋状”突起 ,平均大小 ( 11.8± 3 .9)mm ;1例病灶未能明确显示。使用造影剂后对动脉瘤的诊断符合率 90 % ;造影后相关大脑中动脉的血流速度与造影前相比差异无显著性意义 (P >0 .0 5 )。结论 使用超声造影剂可明显增强颅内血管信号强度 ,提高颅内动脉瘤的检出率。
Objective To evaluate the application of ultrasonic contrast agent Levovist in diagnosis of cerebral aneurysms by transcranial power Doppler sonography. Methods Ten patients with insufficient acoustic windows or without sure diagnosis were selected out from a series of samples who were suspected of aneurysm by transcranial color Doppler sonography. They underwent contrast agent Levovist with continuous injection (2 ml/min). Concentration was 200-400 mg/ml. The diseased area and relevant vascular were observed closely after contrast and parameters data were recorded. The sonographer was blinded to the results of digital subtract angiography and all the cases were confirmed by angiography.Results All the cases showed image enhancement 10-20 s after injection and it lasted 3.5- 9.0 min. Color Doppler images were enhanced obviously. Nine cases showed an obvious “bulge” on the wall of parent artery of aneurysm. Average size of aneurysms was ( 11.8± 3.9)mm measured by color energy Doppler. One out of ten was missed. The diagnostic rate of aneurysms was 90% by using Levovist. In comparison to non-enhanced sonography, the velocity of middle cerebral artery had no significant difference from that of enhanced spectral (P> 0.05). Conclusions Ultrasound contrast agent Levovist improves signal of blood flow obviously. It increases the diagnostic accuracy for intracranial aneurysms and the level of confidence.
