摘要
目的 探讨中心型肥胖和周围型肥胖者空腹、餐后血清肿瘤坏死因子α(TNFα)与胰岛素水平的变化规律。方法 选择中心型肥胖和周围型肥胖患者及正常对照者各 4 5例 ,分别于空腹及餐后抽取肘静脉血 ,以放射免疫法测定血清TNFα及胰岛素含量。结果 3组空腹TNFα水平具显著性差异 ,以中心型肥胖组最高 (P <0 .0 1) ;中心型肥胖组空腹、餐后TNFα水平无显著性差异(P >0 .0 5 ) ,对照组和周围型肥胖组餐后TNFα显著升高 (P <0 .0 1) ;餐后 3组TNFα水平无显著性差异 (P >0 .0 5 )。 3组空腹胰岛素水平无显著性差异 ;餐后中心型肥胖组胰岛素水平明显高于另 2组 (P <0 .0 5 )。结论 中心型肥胖者体内存在着TNFα与胰岛素代偿机制的失调 ,这种失调可能导致和促进胰岛素抵抗。周围型肥胖者无此种现象 ,说明此型肥胖者胰岛素抵抗不明显。
Objective To explore the relationship of serum tumor necrotic factor (TNF) and insulin level in patients of central and peripheral obesity and normal cases during fasting periods and after meals.Methods Intravenous blood was sampled in 45 cases of central obesity,45 cases of peripheral obesity and 45 normal cases during their fasting periods and after meals and then their TNF and the insulin levels were detected through radioimmunoassay.Results There existed significant differences in terms of TNF between the three groups,With the TNF level in the central type group being the highest (P<0.01).No changes in TNF were found during fasting periods and after meals in the central type group (P>0.05).The TNF levels increased significantly in the normal group and the peripheral group (P<0.01).No marked differences were found in the three groups in terms of TNF levels after meals and insulin levels during fasting periods.The insulin levels were found to be higher after meals than those in the other two groups (P<0.05).Conclusion Compensatory function imbalance of TNF and insulin levels occurs in cases of central type obesity,which may contribute to insulin inhibition in these cases but not in peripheral cases.
