伤口急救保护剂对猪股动脉出血的止血研究 被引量:2

Hemostatic effects of aerosol dressing for bleeding from partially ruptured swine femoral artery at first aid

摘要 目的评价伤口急救保护剂对猪股动脉部分断裂的止血效果。方法采用创伤法制成60头猪共60条股动脉的部分破裂,按照保护剂止血使用前是否擦拭伤口及是否辅助止血,分别建立擦拭或不擦拭伤口的止血带法模型、指压法模型和直接喷保护剂法模型等6种止血模型,每种模型均制作10个。术后12h内观察动脉压、心率、止血时间、保护剂膜形成时间、保护剂膜厚度以及保护剂膜是否破裂而继发出血,计算每种模型的止血成功率。结果①使用止血带者,擦拭伤口止血成功率为90%,而不擦拭伤口者为40%;②使用指压法者,擦拭伤口止血成功率30%,不擦拭者为20%;③直接喷保护剂者,擦拭伤口止血成功率为10%,而不擦拭伤口为0。结论对于猪股动脉部分断裂者,采用先扎止血带暂时止血,擦拭伤口后再喷洒保护剂,10min后放松止血带的止血方法成功率最高,是首选的止血方法。 Objective To evaluate hemostatic effects of the wound aerosol dressing (WAD) for bleeding from the partially ruptured swine femoral arteries at first aid. Methods By means of cutting, 6 kinds of bleeding wound models with partially ruptured femoral arteries in 60 swine were established according to the following managements: Directly spray the WAD to the wounds cleaned and uncleaned; hemostasis is done to the wounds cleaned and uncleaned by finger pressing and tourniquet before spraying. Each kind of model consisted of 10 swine. The main indexes, average arterial blood pressure (AABP), heart rate (HR), hemostatic time (HT), the formation time (FT) and the thickness of the WAD membrane shaped while spraying,and the rate of success for each method, were measured within 12 hours postoperatively. Results ①The hemostatic success rates for the models of wounds cleaned and uncleaned, in which hemostasis was achieved by tourniquet, were 90% and 40% respectively. ②The hemostatic success rates for the models with wounds cleaned and uncleaned, in which hemostasis was achieved by finger pressing before spraying, were 30% and 20% respectively. ③The hemostatic success rate for the models, in which the wounds were cleaned before the WAD was directly sprayed to them, was 10%, while the hemostatic success rate for the models in which wounds were uncleaned was 0. Conclusions For the swine bleeding wounds with partially ruptured femoral arteries, the best first aid method is to apply tourniquet firstly to achieve temporary hemostasis, and then clean the wounds and spray the WAD to the wounds, and finally release the tourniquet 10 minutes later. This method has the highest hemostatic success rates in all the 6 methods, so it is the preferred hemostatic method to treat the wounds of partially ruptured arteries and the like.
