摘要
目的 :探讨波及跟距关节跟骨骨折的优选治疗方法。方法 :对 1992年 1月 - 2 0 0 2年 9月收治的 6 2例 (6 6足 )波及跟距关节的跟骨骨折分别采用石膏外固定法、手法复位石膏外固定法、撬拨复位石膏固定法进行治疗 ,并对其疗效进行评估。结果 :所有病例随访 2个月～ 10年 ,Essex LoprestiⅠ型骨折单纯石膏固定法平均得分 96 5 ,撬拨复位法平均得分 98 5 ;Ⅱ型骨折手法复位 +管型石膏固定法平均得分 75 6 ,撬拨复位法平均得分 91 8;Ⅲ型骨折手法复位 +管型石膏固定法平均得分 4 5 6 ,撬拨复位法平均得分 70 3。结论 :Ⅰ、Ⅱ型骨折用撬拨复位可取得较满意效果 ,Ⅲ型骨折手术治疗为首选。
Objective: To investigate the better method for the treatment of calcaneus fracture affected with talocalcaneal joint. Methods: From January 1992 to September 2002,62 cases fracture of calcaneus(66 calcaneal) affected with talocalcaneal joint were treated with different methods,of which clinical effect were evaluated. Results: All the cases were followed up from 2 months to 10 years,fixation with simple gypsum got 96 5 scores and Essex-Lopresti reduction got 98 5 scores with an average in Essex-Lopresti fracture type Ⅰ;manipulative reduction and fixation with tubular gypsum got 75 6 scores and Essex-Lopresti reduction got 91 8 scores with an average in Essex-Lopresti fracture type Ⅱ; manipulative reduction and fixation with tubular gypsum got 45 6 scores and Essex-Lopresti reduction got 70 3 scores with an average in Essex-Lopresti fracture type Ⅲ. Conclusion: In Essex-Lopresti fractures type Ⅰ and type Ⅱ Essex-Lopresti reduction can get satisfactory results,but surgical treatment is the first choice for Essex-Lopresti fractures type Ⅲ.
