摘要
目的 观察氟脲嘧啶 (5 Fu)持续静脉滴注不同时间联合醛氢叶酸 (LV)和奥沙利铂 (L OHP)治疗晚期大肠癌的疗效及毒性反应。方法 经病理组织学诊断的晚期大肠癌 39例。A组 :2 0例 ,L OHP 130mg/m2 静脉滴注 2h ,第 1d ;LV 2 0 0mg/m2 ,静脉滴注 2h ,随后 5 Fu 375mg/m2 静脉推注 10min ,再接 5 Fu3 0 /m2 用输液泵连续滴注 4 8h ,2 1d为 1个周期。B组 :19例 ,方案剂量同A组 ,其中 5 Fu 3 0 /m2 用输液泵连续滴注 12 0h ,2 1d为 1个周期。两组病例至少接受 2个周期治疗。结果 可评价的 35例中 ,A组 18例 ,B组 17例。A、B两组有效率分别为 38 9%、4 2 1% ,经统计学处理 ,差异无显著意义 (P >0 0 5 )。A组毒性大于B组。结论 L OHP +5 Fu +LV方案延长 5 Fu持续滴注时间未能提高疗效 ,却减轻了毒性反应。
Objective To evaluate the efficacy and toxicity of 5 Fu continous infusion for various times combined with LV and L OHP in the patients with advanced colonic cancer.Methods The patients with a histologic diagnosis of advanced colonic cancer were eligible.For group A(20 cases) the chemotherapy regimen was comprised of L OHP 130mg/m 2 infusion for 2 hours,LV 200mg/m 2 infusion for 2 hours followed by 10 minute intravenous bolus of 5 Fu 375mg/m 2,then 48 hours infusion of 5 Fu 3.0/m 2 with an ambulatory pump,repeated every 3 weeks.For group B(19 cases) the chemotherapy regimen was comprised of L OHP 130mg/m 2 infusion for 2 hours,LV 200mg/m 2 infusion for 2 hours followed by 10 minute intravenous bolus of 5 Fu 375mg/m 2,then 120 hours infusion of 5 Fu 3.0/m 2 with an ambulatory pump,repeated every 3 weeks.All patients received at least two cycles of the treatment.Results In 35 patients evaluable,18 in group A,17 in group B,the response rates in both groups were 38.9%,42.1% respectively.There was no statisticant difference between them(P>0.05).The toxicity of group A was more serious than that of group B.Conclusion Prolonging the time of 5 Fu continuous infusin didn't improve the curing effects,but abating the toxicreactions of 5 Fu in the treatment of advanced coloni cancer with LV and L OHP.
