带虹膜膈人工晶状体植入术

摘要 目的 探讨带虹膜膈人工晶状体植入的手术方式及效果。方法 带虹膜膈人工晶状体植入术 3 6例 3 6只眼 ,其中外伤性虹膜缺损伴无晶状体眼 3 1只眼 ,外伤性散瞳伴无晶状体眼 3只眼 ,先天性无虹膜伴先天性白内障 2只眼。明显畏光症状 3 3只眼。手术中带虹膜膈人工晶状体单纯睫状沟植入 13只眼、睫状沟缝线固定 2 3只眼 ,联合抗青光眼手术 3只眼。结果 所有病例完成人工晶状体植入 ,2例术中睫状体少许出血 ,1例在植入过程中人工晶状体襻折断。术后平均随访 18个月 ,最佳矫正视力高于术前矫正视力 3 5只眼 ,畏光症状均有减轻 ,1只眼因玻璃体出血视力低于术前矫正视力。结论 带虹膜膈人工晶状体植入术能提高外伤性无虹膜无晶状体眼视力 。 Objective To review retrospectively the safety and efficacy of black-diaphragm intraocular lenses(IOL)implanted for the treatment of post-traumatic aniridia and congenital cataract with aniridia.Methods 36 eyes of thirty-six patients(mean age 31 years,ranging 18~41)were implanted with a black-diaphragm posterior chamber IOL (Morcher GmbH,Typ 67G)for correction of post-traumatic aniridia associated with cataract or aphakia and congenital cataract with aniridia.This IOL,in poly(methylmethacrylate),consisted of an opaque diaphragm surrounding the transparent optic,and was inserted through a 11-mm corneoscleral limbus,in-the-sulcus implanted(13 eyes),trans-sclerally sutured(23 eyes),combined trabeculectomy in two eyes.Mean follow-up time was 16 months(ranging 8~26).Results Best-corrected visual acuity(BCVA)improved in all eyes except in one.Glare and photophobia decreased in all patients.Intraoperatively,ciliary sulcus bleeding occurred in two cases and haptic rupture occurred during lens insertion in one case.Postoperatively,secondary glaucoma occurred in two eyes,and vitreous hemorrhage in one eye.Conclusion Although in our experience the haptics still seemed weak and the diaphragm diameter too large,implantation of the black-diaphragm IOL Typ 67G appeared sufficiently safe and provided satisfactory functional results for correction of post-traumatic aniridia combined with cataract or aphakia and congenital cataract with aniridia,improving BCVA and reducing glare and photophobia in most patients.Howerer,it is clear that more cases and longer follow-up are needed to assess its clinical performance properly.
