摘要
目的 对非穿透性滤过手术治疗原发性开角型青光眼的疗效进行分析研究。方法 回顾性分析 1997年～1999年在德国Saarland大学眼科医院因药物控制不佳而行非穿透性滤过手术的原发性开角型青光眼病例资料。结果 有 2 16人、 377只原发性开角型青光眼行非穿透性滤过手术 ,平均随诊 31(5～ 5 1)个月。术前平均眼压 (2 8 3±8 4 )mmHg (1mmHg =0 133kPa) ,术后 1周、 6月、 1年、 2年、 3年、 4年眼压分别为 (9 7± 3 6 )mmHg、 (12 4± 5 3)mmHg、 (15 5± 8 7)mmHg、 (15 4± 6 8)mmHg、 (16 1± 6 7)mmHg、 (17 4± 7 5 )mmHg。术前平均视力为 0 6 5 ,术后 1周、 6月、 1年、 2年、 3年、 4年视力分别为 0 6 3、 0 6 8、 0 72、 0 71、 0 76、 0 70。术后 1年显效(不加抗青光眼药物且眼压≤ 2 1mmHg)率为 89% ,总有效 (加或不加抗青光眼药物时眼压≤ 2 1mmHg)率达 98%。术后 4年时 ,显效率降为 4 5 % ,总有效率达 93%。并发症包括切口渗漏、前房出血、脉络膜脱离等。结论 非穿透性滤过手术对药物控制不佳的原发性开角型青光眼治疗效果良好 ,且并发症少。
Objective To explore the clinical effects of non penetrating filtering surgery for primary open angle glaucoma.Methods A retrospective study of primary open-angle glaucoma with bad anti glaucoma drug control and treated by non penetrating filtering surgery in Saarland University (Germany) from 1997 to 1999 was performed.Results There were 377 eyes of 216 patients with primary open angle glaucoma undergoing non penetrating filtering surgery. The average follow up period was 31 (5—51) months. The mean intraocular pressure (IOP) was (28 3±8 4) mmHg (1 mmHg=0 133 kPa) before surgery, and (9 7±3 6), (12 4±5 3), (13 5±5 7), (14 4±5 8), (14 1±6 7) and (16 4±6 5) mmHg at 1 week, 6 months, 1 year, 2 years, 3 years and 4 years after surgery. The mean visual acuity before operation was 0 65, and 0 63, 0 68, 0 72, 0 71, 0 76, 0 70 at 1 week, 6 months, 1 year, 2 years, 3 years and 4 years after the surgery respectively. At first year after the surgery, the complete success (IOP≤21 mmHg without antiglaucoma medications) was 89 % and qualified success (IOP<21 mmHg with or without antiglaucoma medications) was 98 %. At 4th year after surgery, complete success was 45 % and qualified success was 93 %. Complications included wound leak, hypheama and choroidal detachment, etc.Conclusion Non penetrating filtering surgery is efficacious for primary open angle glaucoma uncontrolled by antiglaucoma medications. Few complications occur by the operation.
