摘要
目的 评价含纤维铂金弹簧圈合用聚乙烯醇颗粒 (PVA )行选择性支气管动脉栓塞(SBAE)治疗大咯血的疗效。方法 2 3例患者 ,经微导管用注射器推入含纤维铂金弹簧圈及PVA行SBAE ,术前均行纤维支气管镜明确出血部位 ,术后随诊 2～ 18个月以评价疗效。结果 即时止血成功率为 95 .65 % ,近期复发率为 9.5 2 % ,中远期复发率为 0 ,并发症少而轻 ,对症治疗后缓解。结论 应用含纤维铂金弹簧圈及PVA行SBAE治疗大咯血 ,成功率高 ,适应症广 ,可避免开胸手术 ,是一种治疗大咯血的理想方法。
Objective To evaluate the efficacy of select bronchial artery embolisation(SBAE) used by platinum coils with Dacron fibres and polyvinyl alcohol (PVA) in treatment of massive haemoptysis.Methods Select bronchial artery embolisation(SBAE) were performed by using platinum coils with Dacron fibres and polyvinyl alcohol which pushed by injector through microcatheters.All patients did fiberoptic bronchoscopy before operation,which were to know the site of bleeding definitely.It was follow up 2～18 months to evaluate the efficacy after operation.Results SBAE resulted in an immediate cessation of hemoptysis in 95.65% cases.Short term recurrent ratio were 9.52%.Mid and long term recurrent patients were 0;Complications were few and gently.Conclusion Select bronchial artery embolisation(SBAE) used by platinum coils with Dacron fibres and polyvinyl alcohol (PVA) in treatment of massive haemoptysis,rate of success is high,adaptation is wide,it can avoid to open thorax,it is ideal method to treat massive haemoptysis.
