摘要
目的 对 6 5岁以上老年人行经颅多普勒 (TCD)检测 ,探讨检测老年人双半球脑血流动力学的主要途径。方法 分析 1 1 2 8例老年人 TCD颞窗穿透情况 ,对声窗透声不良者分眼窗、单侧颞窗、单侧颞窗加眼窗三种途径获得双半球脑血流动力学数据。结果 1 1 2 8例老年人 ,颞窗不透声为 1 9.9% (2 2 4例 ) ,其中单侧不透声占 7.8% (88例 ,左侧不透声 4 5例 ,右侧不透声 4 3例 ) ;双侧不透声为 1 2 .1 % (1 36例 )。 2 2 4例透声不良者有2 1例通过单侧颞窗检测到对侧血流 ,占单侧颞窗不透者 2 3.9% ;经眼窗检测 6 1 % (1 36例 ) ;单侧颞窗加眼窗占30 % (6 7例 )。结论 老年人颞窗不透声发生率高 ,透声不良者主要经眼窗检测 ,经单侧颞窗探测对侧血流能提高检出率。
Objective To discuss different approaches to detect hemodynamic changes of bilateral hemispheres in the elder patients(≥65 years) using transcranial Doppler (TCD).Methods One thousand one hundred and twenty eight elder people underwent TCD through temporal window and the data were analysed.For those who had bad windows, bilateral hemisphere cerebral blood flow was obtained from ocular window or unilateral temporal window, or unilateral temporal and ocular windows.Results Among these subjects,224 people (19 9%)had closed temporal window, in which unilateral closed window accounted for 7 8% (88 people, left 45, right 43) and bilateral closed window accounted for 12 1% (136 people) Within these 224 people who had closed temporal window, blood flow of the opposite side can be detected from unilateral temporal window in 21 people (23 9% of unilateral closed window); 61% (136) people were detected through ocular window;30% (67) people through unilateral temporal plus ocular window.Conclusions The incidence of closed temporal window is relatively high in the elders.In the majority of those people who have closed temporal window,the cerebral blood flow can be detected through ocular window,whereas blood flow of the opposite side through unilateral temporal window.Thus,the detection effect may be improved.
