摘要
目的 研究端粒酶在结直肠肿瘤形成、发展过程中的表达规律及其意义。 方法 应用 TRAP与PCR- EL ISA方法检测 30例结直肠腺瘤 ,4 0例配对癌组织和正常黏膜组织端粒酶活性。应用 SPSS1 0 .0统计软件结合临床资料进行分析。 结果 正常黏膜、腺瘤 <1 cm、1~ 2 cm、>2 cm及癌组织端粒酶阳性率分别为 0 ,1 3.3% ,2 5 .0 % ,71 .4 %及 95 .0 % ;端粒酶活性相对水平分别为 0 .0 1 3± 0 .0 0 6 ,0 .0 6 9± 0 .0 89,0 .1 0 2± 0 .0 96 ,0 .35 2± 0 .1 78及 0 .6 5 8± 0 .2 35。从正常黏膜→腺瘤→癌端粒酶阳性率及相对活性水平均逐渐增高 ,差异有显著性 (P<0 .0 5 )。端粒酶活性表达与腺瘤部位及组织学分型无关 (P>0 .0 5 ) ,与腺瘤的大小正相关 (P<0 .0 5 ) ;结直肠癌分化较差 ,Duke's分期较晚的肿瘤端粒酶活性水平较高 (P<0 .0 5 )。 结论 端粒酶激活可能发生于结直肠肿瘤发生的早期阶段 ,并随着腺瘤的增生扩大其表达水平逐步增高直至癌变。端粒酶活性水平可能影响结直肠肿瘤的生物学行为 。
Objective To investgate the changement of telomerase expression in growth and development of colorectal cancer. Methods Telomerase activity was examined by the techniques of TRAP-PCR-ELISA in 40 paired cancer-normal mucosal specimens and 30 adenomatous specimens. For each case, histological diagnosis and telomerase activity were determined on consecutive frozen sections. {Results} Telomerase activity was not detected in mormal mucosa, but 13 3%(2/15) positive in small(≤1 0 cm) adenomas, 25%(2/8) in intermediate adenomas, 71 4%(5/7) in large(>2 0 cm) adenomas, and 95%(38/40) in adenocarcinomas. The level of telomerase activity, in normal mucosa, small adenomas, intermediate adenomas, large adenomas and adenocarcinomas was 0 013±0 006, 0 069±0 089, 0 102±0 096, 0 352±0 178 and 0 658±0 235, respectively. The positive rate or level of telomerase activity was increasing gradually when polyps enlarged or progressed from normal colorectal epithelium to adenoma or carcinomas. The level of telomerase activity in those colorectal carcinomas with poor differentiation or late Duck's stage was higher than well differentiation or early stage. Conclusion Telomerase reactivation occurs at an early stage of colorectal cancers. The level of telomerase activity would influence the biological behavior and may be a good biomarker, the growth and development of colorectal carcinomas.;
