摘要
目的:评价早期应用静舒氧对脑梗死患者运动功能和日常生活活动(ADL)能力的影响及其安全性,为脑梗死的综合治疗提供参考依据。方法:将120例脑梗死患者分为对照组,高压氧组和静舒氧组,3组均按常规治疗配合康复训练,静舒氧组加用静舒氧治疗,每次250mL,2次/d,10次为1个疗程,共2个疗程。高压氧组加用高压氧治疗,将患者置于高压氧舱中,0.2MPa,1h/次,1次/d,10次为1个疗程,共2个疗程。对照组不进行静舒氧及高压氧治疗。应用临床神经功能缺损(NFD)程度评分、简化Fugl-Meyer运动功能积分法(FMA)和Bather指数对3组患者治疗前后神经功能缺损程度、运动功能和ADL进行评价。结果:各组治疗后运动功能和ADL能力评分均有增高,NFD减低,与治疗前比较差异有显著性意义(P<0.05)。组间比较以静舒氧组改善最明显,治疗后FMA评分:上肢(38±9)分,下肢(75±10)分,Barthel指数(83±8)分,高于高压氧组:上肢(29±8)分,下肢(60±10)分,Barthel指数(74±10)分;NFD评分:静舒氧组(13±6)分,低于高压氧组(17±6)分(P<0.05);而高压氧组治疗后FMA评分和Barthel指数又明显高于对照组,NFD低于对照组(P<0.01)。结论:早期应用静舒氧可明显改善脑梗死患者的运动功能及ADL,无明显不良反应。
AIM:To evaluate the effect and reliability of motor function and activities of daily living(ADL) with the treatment of medical pressured oxygen injector at ea rly stage of cerebral infarction(CI) so as to provide references for CI comprehe nsive therapy. METHODS:One hundred and twenty CI patients were randomly divided into control group,hyperbaric oxygen(HBO) group and group treated with medical pressured oxyg en injector.Patients in the three groups received routine treatment associated w ith rehabilitation training.The patients in the group treated with medical press ured oxygen injector received 250 mL oxygen each time,twice a day, ten days as a course,continuously for two courses, while those in the HBO group received HBO therapy in a HBO cabinet with the pressure of 0.2 MPa,one hour each time,once da ily,ten times as a course,totally for two courses.The neurological function defi cit(NFD), simplified Fugl-Meyer motor assessment(FMA) and Barther index(BI) wer e performed among the three groups to evaluate the degree of neurological impair ment,motor function and ADL before and after treatment. RESULTS:The motor function and ADL of the three groups after treatment had hig her scores,but the scores of NFD were lowered.There was a significant difference in the three groups before and after treatment(P< 0.05),especially in the group treated with medical pressured oxygen injector.After treatment, as compared wit h the HBO group,the scores of FMA(upper limbs:38±9 vs 29±8;lower limbs:75±10 vs 60±10) and BI(83±8 vs 74 ±10) were higher and those of NFD(13±6 vs 17±6) were lower in the group treated with medical pressured oxygen injector(P< 0.05) .However,the patients in the HBO group had higher scores in the FMA and BI,and l ower scores in the NFD in comparison with those in the control group(P< 0.01). C ONCLUSION:The motor function and ADL of CI patients are significantly improved a fter the treatment of medical pressured oxygen injector at early stage without s ide effects.
