摘要 目的 :报告Amplatz血栓消融器治疗下肢深静脉血栓的应用情况。方法 :回顾 2 0 0 1~ 2 0 0 2年 18例下肢深静脉血栓 ,腔髂静脉 (n =2 ) ,髂静脉 (n =6 ) ,髂股静脉 (n =9) ,股静脉 (n =1) ,在临时性 /永久性下腔静脉滤器的保护下 ,经患侧静脉 (n =10 )和健侧股静脉 (n =8)为入路行ATD血栓消融术。结果 :13例经ATD血栓消融术后 ,血栓即时清除成功 ,占72 .2 % ,ATD启动平均时间 79.2s,血栓清除率在 2 0 %~ 99% ,平均 70 .3% ,17例患者需辅助治疗 ,占 94 .4 % (17/18) ,12例行Stent置入术 ,13例行经皮血管成形术 ,2例行经导管血栓抽吸术 ,9例行血栓药物溶解术 ,平均随访 6个月均未复发DVT。结论 :在下腔静脉滤器保护下 ,ATD是治疗下肢深静脉血栓的有效方法。 Objective:To study clinical applied value of the Amplatz thrombectomy device (ATD) in the treatment of patients with low limb deep venous thrombosis (DVT).Methods:18 cases with low limb DVT between 2001 and 2002 were reviewed retrospectively.Under temporary or permanent caval filter protection, inferior vencava and iliac vein(n=2),iliac vein(n=6),iliac and femoral vein(n=9),femoral vein(n=1) were treated with the ATD. The ATD was introduced through the ipsilateral popliteal veins(n=10) approach and the contralateral femoral veins(n=8) approach.Results:Primary success was achieved in 13 of 18 cases (72.2%). The mean activation time of the device to obtain the venous patency was 79.2s. The percentage of thrombus removed was 20%~99%,average 70.3%.The complementary procedures were performed in 17 cases (94.4%), Stenting,balloon angioplasty,thrombo-aspiration and medication thrombosis were 12, 13, 2 and 9 cases respectively.Subsequent follow-up was obtained average 6 months after the ATD procedure and without DVT recurrent.Conclusion:Under inferior veno-caval filter protection,ATD is a safe and effective method in the treatment of lower limb DVT.
