摘要
目的 了解皮肌炎患者呼吸中枢肺的功能变化。方法 分别测定了 2 2例皮肌炎患者及 2 0例正常人的用力肺活量 (FVC)、1秒钟用力呼气容积 (FEV1)、最大呼气流量 (PEF)、最大呼气中段流量 (MMEF)、2 5 %肺活量最大呼气流量 (V2 5)、肺一氧化碳弥散量 (DLCO)、最大吸气口腔压 (PImax)及口腔阻断压 (P0 .1)等参数。结果 皮肌炎患者反映肺通气功能指标FVC、V2 5、MMEF及肺弥散功能指标DLCO均明显低于正常人 ,呼吸中枢兴奋性指标P0 .1则显著高于正常人 ,而吸气肌肌力 (PImax)与正常组比差异无统计学意义。结论 皮肌炎患者在肺通气及弥散功能降低的基础上 。
Objective To investigate the changes of central respiratory drive and inspiratory muscle function in patients with dermatomyositis.Methods The measurement of forced vital capacity(FVC),forced expiratory volume in one second(FEV 1),peak expiratory flow(PEF),maximal midexpiratory flow(MMEF),expiratory flow of 25 per cent of FVC(V 25),lung carbon monoxide diffusing capacity(DLco),maximal inspiratory mouth pressure(PImax) and airway occlusion pressure (P 0.1) were performed in twenty-two patients with dermatomyositis and twenty normal subjects.Results In patients with dermatomyositis ,the FVC,MMEF,V 25 which reflect the lung ventilatory function and the DLco which reflects the lung diffusing function were significantly lower than those in normal subjects.Their P 0.1 which reflects the central respiratory drive was significantly higher,compared with the normal subjects.Conclusion Our findings suggest that on the basis of the disorder of ventilation and diffusing function of the lungs ,the central respiratory drive of the patients is increased.
