摘要
目的 探讨脑钠素 (BNP)测定对诊断无ST段抬高的急性冠脉综合征的临床意义。方法 将 5 4例无ST段抬高的急性冠脉综合征患者分为非Q波心肌梗死组 (NQMI组 ,n=2 5 )、不稳定型心绞痛组 (UA组 ,n =2 9) ,分别测定症状发作 6小时内及 2 4小时的BNP和心肌肌钙蛋白T(cTnT) ,并和 30例正常的健康体检者 (对照组 )作比较。结果 症状发作 6小时内 ,BNP含量NQMI组 (5 6 .30± 16 .8) μg/L ,UA组 (35 .30± 15 .4 0 ) μg/L ,均显著高于对照组 (10 .6 4± 1.80 ) μg/L(P <0 .0 1) ,cTnT无显著变化。症状发作 2 4小时 ,NQMI组BNP(15 7.0 0± 2 0 .4 0 ) μg/L和cTnT(9.32± 2 .8) μg/L ,显著高于对照组 (10 .4 6± 1.90 ) μg/L和 (0 .0 3± 0 .0 1) μg/L(P <0 .0 1) ,UA组cTnT有6 3.2 %的患者为正常值 (≤ 0 .1μg/L) ,而BNP显著高于对照组 (P <0 .0 1)。
Objectives To investigate the value of measuring brain natriuretic peptide (BNP) in the patients with non-ST-elevation acute coronary syndromes (ACS).Methods Fifty-four patients with non-ST-elevation ACS were divided into non-Q wave infarction group (group NQMI, 25 cases) and unstable angina group(group, UA 29 cases).BNP and cardiac tropnin T(cTnT) were measured and compared in 54 patients and 30 healthy controls within 6 hours and 24 hours after the onset of symptoms.Results Significantly abnormal concentrations of BNP were observed in the patients with group NQMI[( 56.30± 16.8) μg/L and ( 157.00± 20.40) μg/L (P< 0.01)] and group UA[( 35.30± 15.4) μg/L (P< 0.01)] compared to the controls( 10.46± 1.9) μg/L and ( 10.64± 1.90) μg/L within 6 hours after the onset of symptoms,and the cTnT level was similar in all groups.cTnT and BNP were significantly higher in the patients with non-Q wave infarction ( 9.32± 2.8) μg/L than those in the controls at 24 hours after the onset of symptoms (P< 0.01).BNP was also significantly higher in the patients with unstable angina (P< 0.01).cTnT was normal (≤ 0.1 μg/L) in 63.2% of cases of unstable angina at 24 hours.Conclusion BNP can be used as a diagnostic parameter for early non-ST-elevation ACS.
