摘要
目的:观察椎体成形术中的血液动力学和血气变化及是否发生肺脂肪栓塞。方法:将聚甲基丙烯酸甲酯注入到犬的L2椎体,记录术前、术后颈动脉血压和血气分析值,并将肺组织作病理检查。结果:平均2.6ml的骨水泥注入到椎体后(5±2)s导致颈动脉压下降,持续(150±35)s,并可导致血二氧化碳分压增高和pH值减小,氧分压减低,肺脏病理检查可见到小血管内有脂肪滴。结论:椎体成形术中注射骨水泥可能引起血压下降和肺脂肪栓塞,是椎体成形术的潜在并发症。
Objective:To investigate the hemodynamic and blood gas alterations as well as fat embolism dur-ing vertebroplasty.Method:In four dogs,2.6ml of polymethylmethacrylate was injected into L2.Heart rate,cervical arterial,and blood gas values were recorded before and after injection.The lungs were subjected to postmortem histologic evaluation and compared with lung specimens from two dogs that had not undergone vertebroplasty.Result:Injection of cement elicited a very rapid decrease in cervical arterial pressure(within5±2seconds),last-ed for150±35seconds.The injection resulted in an increase in partial pressure of carbon dioxide and a de-crease in pH and partial pressure of oxygen.The histology of the lung showed intravascular fat globules in lung tissue.Conclusion:The results suggest that cement injection during vertebroplasty,can result in arterial pressure decrease and fat embolism in the lungs that may be a latent complication during vertebroplasty.
