摘要
目的 探讨重型肝炎 (SH)肝组织肝前体细胞 (HPC)的增殖分化特点以及在此过程中肝再生的特点。方法 采用免疫组织化学的方法分别对 5 9例SH病例肝组织标本HPC表达增殖细胞核抗原 (PCNA)的情况进行检测 ;以 5 8例普通肝炎病例作为对照。结果 SH中肝细胞表达PCNA的病例 (30 .5 % )明显低于普通肝炎病例 (5 0 .0 % ) (P <0 .0 5 ) ;而HPC表达PCNA的病例 (4 5 .8% )明显高于普通肝炎病例 (0 % ) (P <0 .0 5 ) ;在PCNA阳性病例中 ,HPC增殖范围半定量为“ -”、“ +”、“ ++”、“ +++”的病例的肝细胞增殖指数分别为 4 6 .96 %± 34.6 3%、2 2 .4 4 %± 37.37%、2 .0 0 %±2 .0 8%、2 .0 1%± 2 .11%。结论 在病毒性肝炎的再生过程中 ,急性肝炎 (AH)和慢性肝炎 (CH)轻度病例主要是以肝细胞的增殖为主 ,在CH中重度有HPC的参与 ,而在SH中 ,肝脏主要是通过HPC的活化和增殖进行再生。
Objective To investigate the features and the probable affecting factors of the proliferation and differentiation of the hepatic progenitor cells (HPC) and the liver regeneration in the patients with severe hepatitis(SH). Methods Liver tissues taken from 59 cases with severe hepatitis were tested for the proliferating cell nuclear antigen (PCNA) by immunohistochemistry (IHC). Meanwhile, PCNA were also detected in liver tissues of 58 cases with common hepatitis as controls. Results The percentage of cases with positive PCNA in hepatocytes in severe hepatitis (30.5%) was significantly lower than that in common hepatitis (50.0%) (P< 0.05); However, the percentage of the cases with positive PCNA in HPCs in severe hepatitis (45.8%) was significantly higher than that in common hepatitis (0%) (P< 0.05). In all cases with positive PCNA, the proliferating rates of hepatocytes were 46.96%± 34.63%, 22.44%±37.37%, 2.00%±2.08% and 2.01%±2.11% respectively. In PCNA positive cases, scopes of proliferating HPCs were defined “-”,“+”,“++” and “+++”. Conclusions In AH and CH(G1-2), the regeneration is mainly owing to thes proliferation of mature hepatocytes; and in CH(G3-4), there is the participation of hepatic progenitor cells, although they are limited. In severe hepatitis, however, the liver regenerates and restores mainly by means of the activation and proliferation of the hepa- tic progenitor cells.
