摘要
目的 评价双对比造影检查中“晕征”及其他各征象对胃粘膜下占位的诊断价值。方法 随机收集经手术和病理证实的胃癌及胃粘膜下占位病例各 10 0例 ,均做过双对比造影检查 ,对“晕征”在两组病例中的分布以及各种征象对胃粘膜下占位病变诊断的敏感性及特异性进行分析、研究。结果 “晕征”在胃癌和胃粘膜下占位两组病例中出现率分别为 2 8%和 95 % ,两组之间差异有显著性 (P <0 .0 1)。“晕征”对胃粘膜下占位病变诊断的敏感性 (95 % )明显高于其他征象 (P <0 .0 1) ,其特异性与雾状影单悬滴、桥形皱襞、猎帽征、牛眼征等胃粘膜下占位的典型征象之间差异无显著性 (P >0 .0 5 ) ;大范围“晕征”在诊断胃粘膜下占位方面的特异性明显高于其他各征象 (P <0 .0 1,0 .0 5 )。结论 双对比造影检查中 ,“晕征”是一种诊断胃粘膜下占位病变的可靠、实用征象 。
Objective To assess the value of helo sign and other double-contrast radiographic signs in diagnosing submucosal lesion of stomach.Methods One hundred cases of gastric carcinoma and one hundred cases of submucosal lesion were chosen randomly.Double-contrast radiography was done in all cases.Distribution of helo sign in two groups cases and sensitivity,speciality of each sign in diagnosing submucosal lesion were analyzed respectively.Results Positive proportion of helosign in gastric carcinoma and submucosal lesion cases was 28% and 95% respectively,significant difference was found between them(P<0.01).Sensitivity of helo sign in diagnosing submucosal lesion was significantly higher than other signs(P<0.01),but speciality was not so(P>0.05).There was significant difference between speciality of large helo sign and other typical signs' in diagnosing submucosal lesion(P<0.01,0.05).Conclusion As a double-contrast radiographic sign,helo sign is reliable and practical in diagnosing submucosal lesion of stomach.It is of great value in distinguishing gastric carcinoma and submucosal lesion of stomach.
