摘要
目的 :对关节盘移位程度进行定量研究并评价移位程度与关节疼痛的关系 .方法 :利用MRI对 4 4例单侧关节疼痛颞下颌关节紊乱 (TMD)患者的 88侧颞下颌关节 (TMJ)进行闭口斜矢状位T1加权成像 .关节盘移位程度的测量方法 :MR成像层面上关节结节最下点 (A)到外耳道最上点 (E)的连线为直线AE ,髁突顶点 (C)及关节盘最后点 (D)与直线AE垂直相交点分别为C′、D′ ,分别求得AD′距离、C′D′距离 ,代表关节盘在关节窝前后方向的位置 .依据可视疼痛模拟标尺 (VAS)值将疼痛侧关节分为轻、中、重度疼痛 3组 ,以测量的AD′、C′D′距离 ,同非疼痛侧关节AD′、C′D′距离做自身对照进行统计分析 .结果 :轻、中、重度疼痛组及无疼痛组AD′和C′D′分别为(8.7± 2 .8)mm和 (1 .2± 3.2 )mm ;(6 .4± 2 .5 )mm和(4 .1± 2 .9)mm ;(7.5± 2 .8)mm和 (2 .2± 3.4 )mm ;(1 0 .9± 1 .7)mm和 (- 0 .9± 1 .7)mm ,统计学分析非疼痛组AD′、C′D′与轻、中、重度疼痛组之间均有显著差别 ;除轻度与中度疼痛组有统计学差别外 ,轻度与重度组间、中度与重度组间AD′、C′D′均无统计学意义 .结论 :TMJ疼痛与盘移位有关 ,但疼痛程度与关节盘前移位程度无相关性 。
AIM: To quantitatively evaluate the degree of anterior disc displacement and its association with temporomandibular joint pain. METHODS: Oblique sagittal T1 weighted MR imaging at closed mouth was obtained from 88 joints of 44 patients who complained of unilateral TMJ pain. On MRI view, a straight line AE was drawn through the lowest point of articular eminence (A) and the highest point of external auditory meatus (E), and then the perpendicular lines were dropped from condylar head (C) and the most poterior band of the disc (D) to AE on which the cross points were marked as C′ and D′. The distance AD′ and C′D′ reflect the anterior posterior position of the disc in the glenoid fossa. The painful joints were divided into mild, moderate and severe pain groups according to the VAS score. The pain free joints served as control. RESULTS: The distance AD′ and C′D′ in mild, moderate, severe and pain free groups were respectively (8.7±2.8) mm and (1.2± 3.2) mm, (6.4±2.5) mm and (4.1±2.9) mm, (7.5±2.8) mm and (2.2±3.4) mm, and (10.9±1.7) mm and (-0.9 ±1.7) mm. Significant difference between all painful groups and pain free group was found. But no significant difference was found between painful groups, except for mild and moderate pain groups. CONCLUSION: Temporomandibular joint pain is associated with anterior disc displacement. But the degree of anterior displacement of the disc has no influence on pain levels, which indicates that the pain level could not reflect the condition of disc displacement.
