窦口鼻道复合体微生态中真菌初步分析 被引量:1

摘要 目的 :初步研究窦口鼻道复合体微生态系真菌分布及临床意义 .方法 :选择成人非真菌性慢性鼻窦炎I型 (S)与II型 (P)患者 4 0例 ,同时设对照组N(2 0例 ) ;在窦口鼻道复合体的 3个不同部位采集标本 (共 1 80个部位 ) ,进行真菌培养鉴定 .分析阳性率、3个部位培养相同率 (SCRF)、优势菌等指标 .同时复习 37例确诊真菌性鼻窦炎的临床资料 ,以比照探讨非真菌性慢性鼻窦炎中真菌的地位 .结果 :N ,S ,P 3组中 ,真菌培养阳性率分别为 1 0 % ,30 %和 1 5 % ,SCRF分别为90 % ,70 %和 85 % ,阳性率及SCRF比较 ,3组差异均无显著意义 (P >0 .0 5 ) ,优势菌为曲霉菌、青霉菌等 .复习 37例中 ,基本都为曲霉菌 ,部位以上颌窦为主 .结论 :对非真菌性鼻窦炎而言 ,真菌在窦口鼻道复合体微生态系中意义有限 ; AIM: To make a preliminary analysis about the fungi in the microecosystem of ostiomeatal complex. METHODS: Forty patients were grouped into S and P; else with 20 control subjects (N). Asepticly, swab speciemens were taken from three different sites in ostiomeatal complex on each individual (totally 180 sites) and sent for fungus culture. Positive culture rate, same culture rate of fungi(SCRF) and predominant species were surveyed. Concurrently, 37 cases with a final diagnosis of fungal sinusitis were reviewed. RESULTS: Positive rate in N, S, P was 10%, 30%, 15%,and SCRF was 90%, 70%, 85% respectively; Differences between the 3 groups were not significant either ( P >0.05). Aspergillus, Penicillium were the predominant species. Almost all that found from the 37 reviewed cases were Aspergillus , and mainly located in maxillary sinuses. CONCLUSION: Under condition that of normal status or nonfungal chronic sinusitis, the role of fungi in the microecosystem of ostiomeatal complex is rather limited and it’s by no means able to evaluate the status and alternations of the certain microecosystem.
