摘要
目的 :观察血滤 (HF)时不同滤膜吸附饱和时间 .方法 :在体外对经LPS刺激后产生大量炎症介质的牛全血进行HF .用数字电桥在线式动态持续监测AN6 9,PMMA ,PS滤膜的电阻值 .用ELISA方法检测各时点血中TNFα水平 .结果 :A ,B ,C三组行血液滤过时不同滤器滤膜的阻值均有随时间变化趋势 (P <0 .0 1 ) ,分别在 1 2 0 ,90和 90min时点阻值最高 ;滤膜的阻值在不同滤器间存在组间差别 ,三组两两比较均为 (P <0 .0 5 ) .不同滤器血中TNFα均有随时间变化趋势(P <0 .0 1 ) ,分别在 1 2 0 ,90和 90min时点血中TNFα浓度最低 ;不同滤器血中TNFα存在组间差别 ,三组间两两比较均为(P <0 .0 5 ) .血滤过程中各时间点血中TNFα的下降与滤膜阻值上升的程度均呈负相关 ,相关系数r分别为 :- 0 .778,- 0 .81 6 ,- 0 .91 5 ,- 0 .96 0 ,- 0 .96 6和 - 0 .95 8,P <0 .0 1 .结论 :用数字电桥 (4位半 )在线式动态持续监测血液滤过时不同滤膜的电阻值可即刻反映滤膜吸附的饱和程度 ,在血流量 2 0 0mL/min ,滤过液流量 2L/h时 ,AN6 9,PMMA ,PS膜饱和时间分别为 1 2 0 ,90和 90min ,AN6
AIM: To observe adsorptive saturation time of different filter membranes in vitro and provide theoretical foundation for the time of changing filter when hemofiltration was carried out. METHODS: 2000 mL cattle blood was collected into one blood bag. Then the blood was stimulated for 12 h at 37℃ with 1 mg/L endotoxin to induce inflammatory mediators. Cattle blood was divided into 3 groups (AN69,PMMA,PS group) and hemofiltration was performed in vitro . AN69,PMMA,PS filter membrane resistance levels were online measured at different time point using Data Bridge. Plasma TNFα concentration was measured by ELISA. RESULTS: Resistance level of AN69, PMMA, PS group presented changing tendency with time going ( P <0.01). The highest resistance level was at 120, 90, 90 min time point in AN69, PMMA, PS group respectively. Resistance level of filter membrane was difference in different group ( P <0.05). Plasma TNFα concentration of AN69, PMMA, PS group also presented changing tendency with time going ( P <0.01). The lowest level of plasma TNFα concentration was at 120, 90, 90 min time point respectively. Plasma TNFα concentration was difference in different group ( P <0.05). TNFα concentration decreasing was significantly correlated with resistance level increasing. r value was –0.778, -0.816, -0.915, -0.960, -0.966 , -0.958 respectively ( P <0.01).CONCLUSION: Membrane resistance level online measured by Data Bridge could instantly reflect the degree of membrane adsorption. Adsorptive saturation time of different membranes was different. Blood flow were 200 mL/min and filtrate flow were 2000 mL/h, adsorptive saturation time of AN69, PMMA, PS membrane were at 120, 90, 90 min, respectively. When adsorption amount of AN69 membrane were the biggest.
