减压后脑肿胀脑水肿与脑循环脑代谢的关系 被引量:8

Correlation of postdecompressive brain swelling and edema with cerebral circulation and metabolism

摘要 用硬脑膜外球囊压迫法造成猫的脑缺血,分别持续5、15及25分钟后减压。连续监测颅内压(ICP)、EEG、脑血流量(CBF)、脑氧代谢率(CMRO_2)及脑动静脉压差(AVDO_2),直至脑死亡或ICP 正常持续24小时。减压后,25分钟脑缺血组立即发生脑肿胀,15分钟组12小时左右发生脑水肿,5分钟组无肿胀和水肿。本结果提示;缺血性脑损害和脑代谢障碍是减压后脑肿胀和脑水肿的基本特点。脑受压时间与脑肿胀和脑水肿发生有密切关系。EEG,CMRO_2及AVDO_2 均可反映脑缺血性损害的程度。 Correlation of decompressive brain swelling andedema with cerebral blood fluid (CBF). cerebralmetablic rate of oxygen (CMRO_2). arteriovenousdifference of oxygen (AVDO_2), intracranialpressure (ICP) and EEG were studied in cats.Global ischemia (flat EEG and no CBF) was createdby controlled expansion of an epidural ballon for5, 15, and 25 minutes respectively in 18 cats. ICPEEG, CMRO_2 were measured continuously untilbrain death or animal survial for 24 hours andICP became normal. After deflation adruptswelling occurred immediately in group of ischemiafor 25 minutes. delayed edema occurred at about12 hours in group of ischemia for 15 minutes, andno swelling and edema found in group of ischemiafor 5 minutes. CBF and CMRO_2 decreased markedly withlow AVDO_2 and flat EEG in swelling and edemagroups. comparing to gradual recovery of EEG.CBF and CMRO_2 with high AVDO_2 in no swellingand edema group (p 0. 05). This study showedthat ischemic insult is basic feature in postdecom-pressive brain swelling and edema. There was aclose relationship between duration of compressiveglobal ischemia and occurrence of brain swellingand edema. EEG. CMRO_2 and AVDO_2 couldthe reflect severity of ischemic insultation.
