摘要
目的 :探讨心理异常与功能性消化不良 (FD)发病的关系 ,并探讨抗抑郁治疗对FD患者的疗效。方法 :采用焦虑自评量表 (SAS)和抑郁自评量表 (SDS)对 10 0例FD患者及 10 0名健康对照组进行心理测评 ,比较 2组焦虑、抑郁的发生率 ;采用评分法对FD患者进行症状评分 ,随机将心理测评异常的FD患者分为常规组及综合组 ,分别给予抗消化不良药物及在此基础上加用抗抑郁药文拉法辛治疗 ,比较 2种治疗方法的总有效率 ;将综合治疗有效的病例随机分成维持组及停药组 ,维持组继续给予文拉法辛维持量治疗 ,观察 6mo中 2组的复发率。结果 :FD组患者焦虑及抑郁状态发生率明显高于健康组 ;综合组有效率高于常规组 ;文拉法辛 6mo维持治疗复发率低于疗程结束后即停药者。结论 :焦虑、抑郁的心理异常是FD发病的危险因素 ,但用抗抑郁治疗可提高FD患者常规治疗的有效率 。
AIM: To discuss the relationship between psyche-disorder and functional dyspepsia(FD) onset and to observe the effect to FD of anti-depression treatment. METHODS: One hundred FD and one hundred healthy volunteers were test by self-rating anxiety scale (SAS) and self-rating depression scale (SDS), the incidence of anxiety and depression were compared between 2 groups; All the patients of FD with abnormal score of SAS and SDS were divided randomly into routine treatment group(only anti-dyspepsia drug be used) and colligation treatment group (anti-dyspepsia drug and venlafaxine be used). The score of SAS, SDS and dyspepsia symptom before and after treatment were compared between 2 groups, the total effective rate of 2 groups also be compared; All the patients with effective treatment in the colligation treatment group were divided randomly into maintenance treatment group (venlafaxine be used continually) and stopping treatment group, the relapse rates after 6 mo were compared between 2 groups. RESULTS: The incidence of anxiety and depression in FD group was higher significantly than that in the health control; The effective rate in the colligation treatment group was higher significantly than that in the routine treatment group. The relapse rate of maintenance treatment group was lower significantly than that of stopping treatment group. CONCLUSION: The anxiety and depression are dangerous factors of FD. Anti-depressive drug(venlafaxine) can improve the effective rate of routine treatment to the patient with FD. Maintenance treatment for 6 mo can decrease the relapse rate of FD significantly.
