摘要
目的 观察慢性重症肝炎膜式血浆置换 (PE)治疗后肝脏生化指标的变化 ,为评价PE治疗慢性重症肝炎疗效提供客观依据。方法 将 71例慢性重症肝炎随机分为对照组和治疗组 ,对照组 (36例 )为内科综合治疗的慢性重症肝炎患者 ,治疗组 (35例 )为内科综合治疗联合PE治疗的病情相同患者。检测PE治疗前后患者肝脏生化指标 (ALT、AST、F Bil、D Bil、CHE、CHOL、ALB、PTA、ALP、NH3 )的变化。结果 PE治疗后各项肝脏生化指标明显好转 ,ALT、AST、T Bil、D Bil、CHE、、ALB、PTA、ALP、NH3 治疗前后相差非常显著 (P <0 .0 0 1) ,CHOL相差显著 (P <0 .0 5 ) ,治疗组治愈好转 2 6例 (74 % ) ,对照组治愈好转 18例(5 0 % )。两组治愈好转率经卡方检验 ,相差显著 (P <0 .0 5 )。结论 内科综合治疗联合血浆置换疗法 ,能显著改善慢性重症肝炎肝功能衰竭患者的临床症状及肝脏生化指标 ,提高治愈率。
AIM To observe the influence of plasma exchange(PE) on liver function indices in patients with severe chronic hepatitis,so as to provide an objective evidence of the therapeutic effect of PE on severe chronic hepatitis.Methods 71 Patients with severe chronic hepatitis were randomly separated into 2 groups,one group taking complex treatment with PE(PE group,n=35),and the other without PE(Control group,n=36). Liver function indices(ALT、AST、T-Bil、D-Bil、CHE、CHOL、ALB、PT、ALP、NH 3、 serum PTA)were observed in both groups before and after treatment,and the treating related changes of clinic symptom were recorded as well.Results After PE treatment,liver function indices were improved obviously,wherein each of ALT、AST、T-Bil、D-Bil、CHE、ALB、PTA、ALP、NH 3 had a very significant difference before and after treatment (P<0.001) and CHOL had a significant difference(P<0.05).In the PE group,26 cases were cured or improved(74%);while in control group 18 were cured or improved(50%),showing a significant difierence between these two groups(P<0.05,by chi square test).Conclusion PE can improve liver function indices significantly,and combined treatmeny with PE can improve recovery rate in patients with severe chronic hepatitis.These results suggest that PE is effective in the treatment of severe chronic hepatitis and hepatic failure through improving liver function indices.
