摘要
目的探讨标准翼点入路治疗颅内前循环动脉瘤的价值。方法对46例前循环动脉瘤应用标准翼点入路开颅,实施显微外科手术,以多种方法处理动脉瘤,以格拉斯哥预后评分(GOS)评定治疗效果。结果43例行动脉瘤夹闭术,2例行包裹术,1例行铜丝栓塞术。除1例术前Hunt-Hess分级Ⅳ级的病人重残外,其余均恢复良好。无死亡,35例随访2～14个月,未发现再出血者。结论标准翼点入路能够充分暴露除大脑前动脉远端外的几乎全部前循环动脉瘤,且操作便捷,损伤小。
Objective To discuss the value of the standard pterional approach for the anterior circulation aneurysms. Methods 46 patients with the anterior circulation aneurysms were treated via standard pterional approach. Glasgow outcome scale (GOS) was conducted to evaluate surgical outcome. Results Aneurysms were clipped in 43 cases, wrapped in 2 cases, and embolized in 1 case respectively. 45 patients gained good recovery except for 1 patient who was preoperative H-H Ⅳ grade and underwent severe disability postoperatively. No patient died of operation. Conclusion The standard pterional approach can provide optimal exposure of the aneurysm and surrounding vital structures along with minimal invasiveness.
