摘要
随着微创外科概念和框架的日益完善,内镜技术作为微创外科的主要手段已在多个领域普遍开展。本文就其在口腔颌面外科各专业的应用现状加以综述,包括:颞下颌关节疾病中关节盘移位、穿孔、囊内粘连、骨关节病、脱位的关节镜治疗;唾液腺疾病中主导管结石的涎腺镜治疗;内镜辅助下各类开放性手术,如下颌下腺、腮腺手术,三叉神经痛、半侧面肌抽搐和舌咽神经痛的微血管减压术,眶周骨、颧骨颧弓、下颌骨髁突骨折的复位-内固定术,LeFortI型、Ⅲ型截骨术、下颌骨牵引成骨术和下颌支垂直切开后退术等正颌手术,骨隆突修整、面部提拉术、咬肌肥大修整、先天性斜颈矫正术,上颌窦、额窦囊肿手术,颈部囊肿摘除和甲状腺肿瘤切除术,上颌窦底增高术等。
Following the development of the concepts and structure of minimal invasive surgery,the endoscopy has been used in lots of area.The application of the endoscopy in oral and maxillofacial surgery has been reviewed in this article,including arthroscopic therapy for disc displacement,perforation,adhesion,osteoarthrosis or dislocation of TMJ,sialendo-scopic therapy for sialolithiasis or stenosis of the duct,and other endoscope-assisted opening surgeries,such as excision of submandibular or parotid gland;microvascular decompression for trigeminal neuralgia,hemifacial spasm or glossopharyn-geal neuralgia;replacement and fixation of the fracture in orbital floor,zygoma or condyle;orthognathic surgery(Le fort I orⅢosteotomy,vertical ramus osteotomy);aesthetic plastic surgery(reduction of prominent bone,facelift,partial resection of masseteric hypertrophy,release of muscular torticollis);sinus surgery for cyst in frontal or maxillary sinus;surgery for cer-vical cyst or thyroid;maxillary sinus floor augmentation.
