谷氨酰胺双肽强化的肠外营养在全胃切除术后的应用 被引量:1

摘要 目的 研究含谷氨酰胺双肽增强的肠外营养对全胃切除术后患者代谢指标、免疫功能和住院时间的影响。方法 68例行全胃切除的胃癌患者,随机均分为对照组和实验组,每组34例。两组患者均接受等氮[0.20 g/(kg·d)]、等能量[30kcal/(kg·d)]的肠外营养7 d,实验组接受谷氨酰胺双肽的补充,相当于谷氨酰胺0.34/(kg·d)。在术前和术后第1、8天取血测定血常规、血糖、肝肾功能、CD3、CD4、CD8、CD4/CD8比值、淋巴细胞转换率,并对患者的住院时间进行观察。结果术后第8天,实验组CD4、CD4/CD8、淋巴细胞转换率均显著高于对照组(P<0.01)。实验组患者手术后住院时间显著低于对照组(P<0.01)。结论含谷氨酰胺双肽的肠外营养可改善全胃切除术后患者的免疫功能,缩短住院时间。 Objective To evaluate the effects of parenteral nutrition with alanyl glutamine-supplementation on metabolic index, immune function and hospital stay in patients after total gastrectomy. Methods Sixty-eight gastric cancer patients who had undergone total gastrectomy were divided into experiment group and control group at random. All patients received equal nitrogen [0.20 g/(kg·day)] and calorie [ 30 kcal/(kg·day)] for 1 week.The patients in experiment group were given alanyl glutamine, which was equal to glutamine 0.34 g/(kg·day) . Blood samples were taken for measurement of blood routine, sugar, hepatic and renal functions, CD3, CD4, CD8, CD4/CD8 and lymphocyte blastogenesis test (LBT) preoperatively and on day 1 and day 8 after operation.The hospital stay was also noted. Results On day 8 after operation , CD4, CD4/CD8 and LBT in experiment group were significantly higher than those in control group ( P<0.01) . The hospital stay for experiment group was significantly shorter than that for control group ( P < 0.01) . Conclusion Parenteral nutrition supplemented with alanyl glutamine improves immune function and shortens hospital stay for patients after total gastrectomy.
