Dig-McAb Fab片段逆转猴严重地高辛中毒及其对药物动力学影响的观察

摘要 用Dig-McAb (digoxin-specific monoclonal antibody,地高辛单克隆抗体)Fab片段和IgG分别救治猴严重地高辛中毒成功。对照猴用无关McAb救治则短时内死于心室颤动。救治成活猴血清地高辛含量于Dig-McAb生效时,成倍增高且半衰期延长。Fab片段治疗后血中无相应抗抗体存在。表明Dig-McAb Fab片段是解除猴严重地高辛中毒之安全、速效而可靠的解毒剂。 To test further their efficacy in reversal of advanced, otherwise lethal digoxin toxicity and investigate the possible immunological adverse effects on patients in clinical practice, digoxin specific monoclonal antibody Fab fragments (dig-McAb) and IgGs were used to treat monkeys with threatening digoxin-toxic arrhythmias. All monkeys survived. Detoxificative effects were produced only 5 minutes after giving Dig-McAb intravenously in a dose twice the chemical equivalent of digoxin and the ECG became normal in 5 hours. Control animal given nonspecific monoclonal antibody IgGs after the onset of arrhythmias died in less than 30 minutes. Plasma digoxin concentrations measured by RIA increased muhifold at the time of recovery and the half-life of digoxin in plasma was markedly prolonged, especially in animal treated with Dig-McAb. No anti-Dig-McA-Fab was detected with double immunodiffusion in plasma obtained on the 21th and 40th day during the observation period. It is concluded that digoxin monoclonal antibody Fab fragments are safe, reliable and effective antidote for severe digoxin toxicity.
