摘要
目的 观察诺维本治疗恶性胸腔积液的疗效 ,探讨与生物化学联合疗效。方法 把患者随机分为诺维本治疗组 (2 6例 )、重组人白细胞介素 - 2治疗组 (2 3例 )及两药联合治疗组 (2 4例 )。均行胸腔闭式引流术 ,排尽胸水 ,注入诺维本 30 mg/ m2 ,重组人白细胞介素 - 2 2 0 0万 IU/ m2 ,用 2 %利多卡因 5～ 10 ml溶解、稀释 ,夹管 ,患者变动体位 ,2 h后松开引流管持续引流 ,当胸水引流量 <5 0 ml/ d经胸透或超声证实基本无胸水后拔管。结果 诺维本组有效率 88.5 %、重组人白细胞介素 - 2组有效率 5 2 .2 % ,两组疗效差异有显著意义 ,而联合用药组有效率 91.7% ,与诺维本组疗效差异无显著意义。三组副作用无明显差异 (P>0 .0 5 )。结论 诺维本治疗恶性胸腔积液效果显著 。
Objective To evaluate the effects of vinorelbine on the treatment of malignant pleural effusions patients, using a combined method of bio-chemical therapy.Methods All patients were randomly divided into three groups: vinorelbine group (n=26), rhIL-2 group (n=23) and combined treatment group (n=24).All cases were performed by closed thoracic drainage before instilled drugs. The vinorelbine 30g/m 2, rhIL-2 2.0×10 6IU/m 2 were dissolved respectively in 2% lidocaine solution 5~10ml. After instilled drugs, patients were asked to change position every 15 min for 2 hours. When the drainage level decreased to <50ml/d and when B-ultrasonic wave or thorax radiography showed its completion, the chest tube was removed. Results The effective rate was 88.5% and 52.2%, respectively for the vilrelbine group and for the rhIL-2 group: p<0.05. In the combined treatment group, the effective rate was 91.7%. The effective rate of the combined treatment group was not significantly different from the vinorelbine group. There were no marked difference among the three groups’ side reaction (p>0.05).Conclusion Vinorelbine is a highly effective treatment for the malignant pleural effusion patients, while the combined treatment of bio-chemical therapy isn’t significantly better than vinorelbine.
