摘要
目的 :评价Nd :YAG激光术治疗慢性肥厚性鼻炎的临床应用价值。方法 :将 2 5 4例患者随机分为两组 ,分别应用下鼻甲部分切除术和Nd :YAG激光术进行治疗。下鼻甲部分切除术组118例 ,Nd :YAG激光术组 136例。结果 :术后 6个月随访 ,下鼻甲部分切除术总有效率 88 98% ,Nd :YAG激光术总有效率 91 91% ,二者无显著性差异 (χ2 =0 6 3,P >0 0 5 ) ;前者术后抽取纱条出血者 18 6 4% ,后者为 0 74 % ,有极显著性差异 (χ2 =2 2 4 8,P <0 0 1)。两种治疗术后出现的鼻腔干燥和粘连比较均无显著性差异。结论 :下鼻甲部分切除术与Nd :YAG激光术治疗慢性肥厚性鼻炎效果相似。但Nd :YAG激光术具有手术过程简单、时间短 ,容易操作、鼻甲黏膜损伤小、术中及术后出血少、术后反应小等优点。
Objective:To evaluate the curative effect of the treatment of chronic hypertrophic rhinitis with Nd:YAG laser and partial inferior turbinectomy..Methods:All cases were randomly divided into two groups.136 patients with chronic hypertrophic rhinitis were treated with Nd:YAG laser operation and 118 patients were received treatment of partial inferior turbinate at our hospital..Results:Follow-up observation on the postoperative 180 day found the total effective rate of partial inferior turbinectomy was 88.98% and that of YAG laser surgery was 91.91%.There was no remarkbale difference between the two groups(χ2=0.63,P>0.05).The postoperative bleeding incidence in partial inferior turbinectomy group was 18.64% and that in YAG laser surgery group was 0.74%.There was remarkable difference between the two groups(χ2=22.48,P<0.01).Conclusion:The effective rates of partial inferior turbinectomy is similar in treating chronic hypertrophic rhinopathy to Nd:YAG laser operation,but the later has several advantages such as simple operative procedure,shorter operative time,less nasal mucosa damage,less hemorrhage,easy to operate and so on.
