摘要
本文就甘露醇在降颅内压时的静脉输注剂量及速度进行了研究。10例颅内高压病人分为二组,高剂量组5例,静脉输入20%甘露醇760±50mg/kg,k为86.36mg/kg/min,低剂量组5例,静脉输入20%甘露醇400mg/kg,k为53.33mg/kg/min。结果表明,用药后20—30min两组病人脑压均下降到最低值,下降百分率分别为:高剂量组19.58±0.05%,低剂量组22.64±0.11%。高剂量组用药后1h脑压明显升高,出现明显的反跳现象,用药后230min达最高值,反跳百分率为41.04±0.71%,而低剂量组无反跳现象。提示:当静注剂量不超过400mg/kg,输注速度不超过50mg/kg/min时可以防止反跳现象。
The dosage and rate of intravenous infusion during mannitol treatment of patientswith raised intracranial pressure were studied. 10 patients were divided into two groups. 760±0.05mg/kg of 20% mannitol were administered to 5 patients at a rate of 83.36mg/kg/min in high dosegroup,and 400 mg/kg to other 5 patients at a rate of 53.33 mg/kg/min in low dose group. The results showed that at 20 to 30 minutes after infusion the intracranial pressure of patients reduced to it's low-est value. The percentage of intracranial pressure decrease were 19.58±0.05% in high dose groupand 22.64±0.11% in low dose group respectively. In high dose group, 1 hour after administrationthe intracranial pressure of patients began to increase. An evident phenomenon of rebound overshootoccurred and 230 minutes after administration the intracranial pressure of patients increased to its high-est value. The percentage of the intracranial pressure increase in rebound overshoot was 41.04±0.17%,but there was no rebound overshoot phenomenon in low dose group. These indicated that the re-bound overshoot phenomenon of mannitol could be prevented when the dosage was not higher than 400mg/kg and the infusion rate was slower than 50 mg/kg/min.
