摘要
探讨风心病房颤病人心房间质胶原蛋白Ⅰ、Ⅲ的基因表达改变,以进一步明确房颤的机制,阐述心房结构重构与风心病慢性房颤的关联。用RT-PCR方法检测24例风心病并发房颤病人与20例风心病窦性心律病人中胶原蛋白Ⅰ、Ⅲ的mRNA表达的差异;同时利用免疫组化及图像分析方法检测心房间质中Ⅰ、Ⅲ型胶原含量并作对比分析。风心病并发房颤病人胶原蛋白Ⅰ、ⅢmRNA表达及胶原蛋白含量均较风心病窦性心律病人组明显增高,二者呈正相关。胶原增生,心房重构等结构改变与风湿性心脏病房颤的发生与维持密切相关并且不同类型胶原增生程度存在差异。
To investigate the difference of collagen Ⅰ, Ⅲ (col Ⅰ, Ⅲ) gene expression in right atrial extracellular matrix (ECM) of patients who suffer from rheumatic heart disease in chronic atrial fibrillation (AF) or sinus rhythm (SR), elucidating the relationship between atrial fibrillation and structure remodeling. Right atrial free wall were obtained from 24 patients in chronic atrial fibrillation (AF) and match with 20 patients in sinus rhythm. Total RNA was isolated and the mRNA expression of collagen Ⅰ, Ⅲ were measured by reverse tran-scription-polyerase chain reaction ( RT-PCR) and localized by immunohistochemistry and analysed by VDS. The mRNA expression of collagen Ⅰ, Ⅲ were significantly higher in patients in AF than those in SR(SR: Col Ⅰ/GAPDH 0. 74±0.23, ColⅢ/GADPH 0.36±0. 12; AF: Col/GAPDH 1.61±0. 54, ColⅢ/GADPH 0. 56±0. 21 ) and were correlated positively with those results of immunohistochemistry. Enhanced collagen gene expression may cause increase in collagen synthesis and expression level were different between Col I and Col Ⅲ in right atrial walls of patients in AF; The increase of collagen synthesis and atrial structure remodelling may be involved in the incidence of chronic atrial fibrillation in patients with rheumatic heart disease.
