高压氧对家兔肝脏缺血再灌注损伤的保护作用 被引量:13

摘要 目的 探索高压氧 (HBO)对肝缺血再灌注损伤 (I/R )的作用及其干预时相。方法 阻断兔入肝血流 2 0min后再灌注。术后分为对照组和不同时期HBO处理组 ,观察动物苏醒时间、不同时相谷丙转氨酶 (ALT) ,超氧化物歧化酶 (SOD )和丙二醛 (MDA)的变化 ,于第 11天取活体肝脏组织行电镜观察。结果 HBO立即组的ALT恢复较快 ,与对照组差异有显著性 (P <0 .0 5 )。高压氧处理组的SOD水平高于对照组 (P <0 .0 5 )。各高压氧处理组的MDA水平与对照组比较无显著性差异 (P >0 .0 5 )。电镜观察结果 :HBO立即组 ,HBO 2 4h组 ,HBO 72h组和对照组的肝细胞凋亡数分别为 :偶见/片 ,2个 /片 ,5个 /片和 10个 /片 ;细胞水肿程度以立即组最轻 ,对照组最重。结论 HBO对肝I/R具有保护作用 ,处理越早作用越明显。 Objective To determine the protective effect of hyperbaric oxygen(HBO) on the liver ischemic reperfusion (I/R) injury and when the treatment should be used.Methods The rabbit hepatic inflow was occluded for 20 min,and then the occlusion was released for reperfusion. The rabbits were divided into a control group and different time(immediate,24h,72h after the 1/R injury induced) HBO therapy groups. After the operation,the waking, ALT, SOD and MDA of the rabbits were observed regularly. The hepatic tissue was observed with electronic microscope on the 11th day after the operation. Results The ALT in the immediate HBO therapy group recovered significantly faster than the control group(P<0.05). The SOD level in the HBO group was higher than that in the control group(P<0.05). Campared with that of the control group,the MDA level in all HBO therapy groups showed no significant difference(P>0.05). The numbers of apoptosis in the immediate group, 24h group, 72h group and the control group were occasionally seen/film, 2/film, 5/film and 10/film,respectively. The lightest edema of cells was seen in the immediate group and most serious in the control group. Conclusions Hyperbaric oxygen therapy has good protective effect on liver ischemic reperfusion injury,and the earlier the HBO is used, the better the protective effect.
