脱细胞真皮基质在整形美容外科中的应用 被引量:8

Application of acellular dermal matrix in cosmetic plastic surgery

摘要 目的 为改善皮肤移植的形态和功能 ,提高大面积烧伤缺乏自体供皮来源患者的美容整形治疗效果。方法 应用自制脱细胞真皮基质 (ADM ) ,植入松解或切除瘢痕后的创面 ,5~ 10d后再行自体刃厚皮移植 ,术后观察ADM和创面愈合、瘢痕增生及功能恢复情况。 3例再次手术患者留取复合移植组织进行组织学检测。结果  15例 2 3次复合皮移植 :14例 2 2次成活率为 10 0 % ,1例 1次成活率为80 % ,植皮区平整光滑 ,瘢痕增生不明显 ,组织学检测提示复合皮血管化明显 ,胶原排列整齐 ,表皮 真皮连接结构重建充分 ,未见排异反应。结论 ADM与自体刃厚皮复合移植能改善创面愈合的质量 。 Objectives To improve the cosmetic appearance an d function of the grafted site especially in case of insufficient donor sites av ailable. Methods The allogenic acellular dermal matrix (ADM) w as used for covering the defect after scar release or excision, then overlying i t with split thickness autograft. The wound healing, scar developing and joint f unctions recovery were observed at different time after operation. 3 wound tissu e samples that were re-operated were harvested for histological examination. Results The wound was completely healed in 14 cases (22 sheets); Only one patient was partly healed (80%). There was only slight contraction, the surface was smooth, the consistency was soft, and the function was good. It was indicated by histological examination that there was obviously vascularization, orderly collagen proliferation and reconstruction of epithelial-dermal conjunt ion structure. There was no obvious sign of immune rejection. Conclusion The wound covering with recombinant composite skin graft of human epit helium and ADM improves wound healing quality. ADM is an optical repairing mater ial in cosmetic plastic surgery.
