原发性高血压发病机制中红细胞内Ca^(2+)浓度及红细胞膜Ca^(2+)-Mg^(2+)-ATP酶活性变化与左心室肥厚的关系(英文)

摘要 背景:左心室肥厚(left ventricular hypertrophy,LVH)患者红细胞内游离Ca2^+浓度、红细胞膜Ca2^+-Mg^2+-ATP酶活性的变化尚不清楚。 目的:探讨原发性高血压(essential hypertension,EH)患者伴LVH和不伴LVH红细胞内游离Ca2^+浓度、红细胞膜Ca2^+-Mg2^+-ATP酶活性的变化及其相互关系,揭示了原发性高血压的发病机制,为康复措施的介入提供理论依据。 设计:以诊断为依据的病例对照研究。 地点、对象和方法:检测30例来源于解放军第四军医大学唐都医院健康体 检自愿参加研究的正常血压者(正常血压组);82例来源于本院的轻度和中度EH患者(高血压组)(其中LVH组40例,非LVH组42例)外周血红细胞内Ca^(2+)浓度及红细胞膜Ca^(2+)-Mg^(2+)-ATP酶活性。 主要观察指标:血压正常者和EH患者外周血红细胞Ca^(2+)浓度和红细胞膜Ca^(2+)-Mg^(2+)-ATP ase活性测定结果比较;LVH和非LVH患者性别,年龄,体质量指数,心率,收缩压,舒张压,平均压,IVST,PWT,EDD,LVMI,外周血红细胞内Ca^(2+)浓度,红细胞膜Ca^(2+)-Mg^(2+)-ATP酶活性测定结果比较;EH患者LVH与Ca^(2+)的相关性。 结果:①EH组外周血红细胞内Ca^(2+)浓度显著高于正常血压组[(1013.5±90.4),(1286.1±95.7)nmol/L,P<0.01],红细胞膜Ca^(2+)-Mg^(2+)-ATP酶活性明显低于正常血压组[ BACKGROUND:The changes of intracellular free calcium and cell membrane Ca2+-Mg2+-ATPase activity of erythrocyte in essential hypertension (EH) patients with left ventricular hypertrophy (LVH) remain unknown.OBJECTIVE:To investigate the relationship between intracellular free calcium and cell membrane Ca2+-Mg2 +-ATPase activity of erythrocyte in EH patients with and without LVH and to reveal the pathogenesis of EH,so as to provide a theoretical basis for rehabilitation intervention of EH.DESIGN:Diagnosis-based case-control study. SETTING,PARTICI PANTS,and METHODS:We recruited 30 voluntary nonnotensives and 82 EH outpatients (40 with LVH and 42 without LVH) from Tangdu Hospital,Fourth Military Medical University.The intracellular free calcium level of erythrocyte was determined by flow cytometry using a new indicator named Flo-3/AM.Cell membrane Ca2+-Mg2+-ATPase activity of erythrocyte was measured by a colorimetric method.RESULTS:(1)The intracellular free calcium level of erythrocyte in EH patients was remarkably higher than that in nonnotensives [(1 286.1±95.7) and(10 13.5±90.4) nmol/L,t=-5.64,P < 0.01],with significantly lower membrane Ca2+-Mg2+-ATPase activity[(21.53±4.28) ukat/g and(115.7±19.1) u.molPi/gHb.h,t=4.86,P<0.01].(2) Compared with the non-LVH group,the LVH patients had significantly higher intracellular free calcium level and significantly lower membrane Ca2+-Mg2+-ATPase activity (P<0.01).(3)LVMI had a positive correlation with intraerythrocytic Ca2+ level (r=0.902,P<0.01),but a negative correlation with the activity of erythrocyte membrane Ca2+-Mg2+-ATPase (r=-0.683,P<0.01). CONCLUSION:EH patients with LVH have higher intracellular free calcium level and lower membrane Ca2+-Mg2+-ATPase activity of erythrocyte,suggesting that intracellular free calcium overload may be one of the mechanisms leading to LVH.
